| Patterson 538.  Currently drilling at 4426' on SOBM.  Walked site.  Poly tank on back of location with vent pupe.  Can hear gurgling in tank.  According to Co. man they are venting all three annuli of 3H, and surface and intermediate of 2H.  Requested operator to isplate each annulus and collect pressure reading and/or flow information to determine what is cause of noise in tank.  Cement records provided indicate cement returns on both surface and intermediate casing strings.  20" set at 140'.  13 3/8" set at  709', and 9 5/8" set at 1532'.

| I inspected the Burts Well Site on March 26, 2018.  The access road was a bit muddy where it passes through a quarry, but was solid enough overall. The pad surface was solid and partly snow covered. The cut and fill slopes were well vegetated, visible through the snow.  There were no areas of instability noted.  The wells were properly labeled, and the wells on site were as follows:
 +
115-20669
 +
115-20707
 +
 +
115-20668 (plugged)

| Patterson 538. Currently drilling at 4426' on SOBM. Walked site. Poly tank on back of location with vent pupe. Can hear gurgling in tank. According to Co. man they are venting all three annuli of 3H, and surface and intermediate of 2H. Requested operator to isplate each annulus and collect pressure reading and/or flow information to determine what is cause of noise in tank.   Cement records provided indicate cement returns on both surface and intermediate casing strings. 20" set at 140'. 13 3/8" set at  709', and 9 5/8" set at 1532'.

2018661 Primary Facility 11/07/2011 Follow-up Inspection No Violations Noted Patterson 538 being rigged up. Recieved call from jerry Dugas of Cabot on 11/3/11 - report of gas detected in cellar when conductor set. Selman on site with Sensit HXG3 gas meter. Checked cellar and conductor with meter - no detectable levels of combustible gas detected. Secondary containment in place - have interlocking mats on site. No violations.
2027652 Primary Facility 12/06/2011 Drilling/Alteration No Violations Noted Patterson 538.
2408433 Primary Facility 09/21/2015 Routine/Complete Inspection No Violations Noted
2451737 Primary Facility 2016-01-21 Routine/Complete Inspection No Violations Noted The Department was on site to inspect the Cabot L. Burts 3 well site (ESX11-115-0074). The wells asociated with this site are as follows:

115-20707 115-20668 115-20669

This site is located in Forest Lake Township, and its last company E&S inspection was on 1-19-2016. The vegetation on site was very well established and there was no erosion or sedimentation issues at this time.

There were no violations.

2505616 Primary Facility 2016-07-27 Routine/Complete Inspection No Violations Noted At time of inspection, arrived on site at 10:20a.m., the cellar was full of water with no bubbling noticed, the (13x9) vent pipe was open and venting into a tank, the (9x5) vent pipe was open and venting into a tank, there was 556psi on the production casing, I took a walk around the location and everything looked OK.

I left the site at 10:50a.m.

2599616 Primary Facility 2017-05-31 Routine/Complete Inspection No Violations Noted At time of inspection, arrived on site at 12:35p.m.

The cellar was full of water with no bubbling noticed. The (13x9) annulus vent pipe was open and venting into a small plastic tank, the (9x5) annulus vent pipe was open and venting into a small plastic tank. There was 492psi on the production casing. I took a walk around the location and there were no obvious concerns noticed. I left the site at 13:00p.m.

2705282 Primary Facility 2018-03-13 Routine/Complete Inspection No Violations Noted At time of inspection.

The well was in production. There was water and ice 1' below grade in the cellar and there was no bubbling noticed. The (13x9) and (9x5) annuli vent pipes were connected together and going to the same tank. I took a walk around the location and there were no obvious concerns noticed. There were no violations noted.

2709987 Primary Facility 2018-03-26 Routine/Complete Inspection No Violations Noted I inspected the Burts Well Site on March 26, 2018.

115-20669 115-20707

115-20668 (plugged)

There were no violations noted

2805398 Primary Facility 2018-11-20 Routine/Complete Inspection No Violations Noted At time of inspection.

The well was in production. There was water 4' below grade in the cellar with no bubbling noticed and there was no gas detected in the remainder of the cellar. The (13x9) and (9x5) annuli vent pipes were connected together and going to the same poly vent tank. There was gas detected at the poly vent tank with a MSA Altair 5X multi gas detector that was calibrated on 11/2/18. I took a walk around the location and there were no obvious concerns noticed. There were no violations noted.
