| I performed an inspection of the Plonski pad.  The weather was overcast and about 32 degrees.
|
+

|
+
The access road was clear of snow and was just slightly wet, without ruts or runoff problems.  The fill slope was visible from the access road and was well vegetated and appeared stable.  No rills or ruts were visible.
|
+

 +
The pad surface was partially plowed and was solid.
 +

 +
The cut slope was well vegetated and stable.
 +

 +
The site was stable overall and no erosional problems were apparent at the time of inspection.
 +

 +
The wells were all clearly labeled.

Inspection Data

Inspections Performed

Inspection ID Inspection Category Inspection Date Inspection Type Inspection Result Inspection Comments
2065314 Primary Facility 05/01/2012 Drilling/Alteration No Violations Noted Diaz spud rig on site. Conductor has been set. Operator provided permit while on site. Crew on site hydroseeding banks of pad. No violations observed.
2072752 Primary Facility 05/21/2012 Routine/Partial Inspection No Violations Noted
2072978 Primary Facility 05/24/2012 Routine/Partial Inspection No Violations Noted Areas downslope of FS#8, which should be left undisturbed, have been cleared. Waste has been noted along the north and northwestern areas of the topsoil stockpile.Violations associated with this inspection have been attached to the J Plonski 1 Well (Permit # 115-115-20337)
2085158 Primary Facility 07/19/2012 Drilling/Alteration No Violations Noted H&P 371. Company Man - Sam Morgan. Ran casing to 11328' Got hung up and had to TOH. Centralizers show excessive wear. Cleaning out hole and re-run casing. Drillers log on site and up to date. Asked Co. Man to check on containment around diesel storage tank. Also requested to restore containment on back corner of pad near dry material storage area. TD= propsed 13425'.
2233393 Primary Facility 01/02/2014 Drilling/Alteration No Violations Noted
2348442 Primary Facility 02/24/2015 Routine/Complete Inspection No Violations Noted
2477159 Primary Facility 2016-04-21 Routine/Complete Inspection No Violations Noted At time of inspection, arrived on site at 12:35 p.m., the cellar had water at the bottom with no bubbling noticed, the (13x9) vent pipe was open and venting to a tank, the (9x5) vent pipe was open and venting to a tank, there was 631psi on the production casing. I took a walk around the location and everything looked OK.

I left the site at 1:15 p.m.

2748187 Primary Facility 2018-07-02 Routine/Complete Inspection No Violations Noted This inspection was conducted to confirm Cabot's assessment that the project meets the minimum requirements for permit termination. The Department’s determination is that the site meets the requirements for permit termination. I recommend the NOT be approved.
2852401 Primary Facility 2019-03-12 Routine/Complete Inspection No Violations Noted I performed an inspection of the Plonski pad. The weather was overcast and about 32 degrees.

The access road was clear of snow and was just slightly wet, without ruts or runoff problems. The fill slope was visible from the access road and was well vegetated and appeared stable. No rills or ruts were visible.

The pad surface was partially plowed and was solid.

The cut slope was well vegetated and stable.

The site was stable overall and no erosional problems were apparent at the time of inspection.

The wells were all clearly labeled.
