The inspection was conducted as a result of an operator request in accordance with Section 3258(a.1) of Act 13, The Oil and Gas Act. Mr. Jamie Davis representing EQT Production Company was also present during the inspection.
The Act 9 sign was clearly legible at the site entrance and all required documentation was available on-site. The access road and the well pad were in good condition. At the time of the inspection, stone was being graded onto the well pad. Upon finishing the well pad, more stone will be added to the access road. All E&S BMPs were installed and appear to be functioning as intended. At the time of the inspection, a crew was activly working along the perimeter maintaining the E&S controls. The operator has restored the site to the interim restoration plan and has constructed the PCSM BMPs. A slope failure in the well pad fill slope has developed. The failing slope has been repaired by the operator previously, but continues to move. The slope failure has not impacted the well pad and has not yet left the approved LOD. The slope failure is encroaching on the LOD and has begun to impact the perimeter E&S BMPs. The operator will need to stabilize the area. The operator will need to complete a geotechnical investigation and report for repair of the slide and stability of the site. 6 wells are have been permitted on the site. Drilling operations are ongoing on the laterals for these 6 permitted wells. |
