| On location at 11:30 A.M. to witness the testing of the blow out preventer (BOP).  Poulson Drilling closed the BOP at 400 psi and charged the annulus to 180 psi.  Pressure held for 15 minutes with no observable leak off.  The test passed and Poulson Drilling can drill out from under the surface casing.  The pits were in good condition and had plenty of freeboard.  No violations.

| On location to witness pit construction.  The bedrock was encountered at 3-4 feet below the topsoil.  I had Poulson Drilling install road fabric in the bottoms of the working and reserve pits prior to installation of the pit liner to prevent puncturing of the liner.  No violations.

| On location at 1:00 A.M. to witness cementing of the 8.625 surface casing.  Poulson Drilling ran 6 joints of new 8.625 surface casing.  Petroset rigged up, established circulation, and cemented 264 feet of surface casing with good cement returns to the surface.  Poulson Drilling was told to wait 8 hours for the cement to thicken and reach an appropriate compressive strength before breaking off.  See the corresponding cement report for further details.

| I was on location to check if a gate had been installed on the lease road.  Steel poles had been installed on each side of the road, but the gates had not been put in.  I called Alan Lorinchack (Summit Petroleum) and asked to get the gates installed within a week.  He said he would take care of it.  I will check back next week.  No violations.

| During a status check of this well I found that the posts for the lease road gate had been set off of State Route 82.  Posts had been placed around the pumping unit and were awaiting the fencing to be placed on them.  The area around the well has been reclaimed, seeded, and is beginning to fill in with grass and clover. The tank battery for this well is located at the Craver-Nedalka #1.  No violations.

| I performed this status check inspection to check the progress of the installation of the surface equipment following completion of the well the previous week.  I found a pumping unit on location and it had not yet been spotted over the wellhead.  A trailer with used rods and tubing was on location waiting to be run into the hole.  Four frac tanks and one flow back tank were still present on the  location.  The lease road was in good condition.

| On location at 1:00 A.M. to witness cementing of the 8.625 surface casing.  Poulson Drilling ran 6 joints of new 8.625 surface casing.  Petroset rigged up, established circulation, and cemented 264' feet of surface casing with good cement returns to the surface.  Poulson Drilling was told to wait 8 hours for the cement to thicken and reach an appropriate compressive strength before breaking off.  See the corresponding cement report for further details.

Inspections Performed

Inspection ID Inspection Date Inspection Type Inspection Result Inspection Comments
0073191732 2012-07-21 Urban Drill / Deepen / Reopen No Violations On location at 11:30 A.M. to witness the testing of the blow out preventer (BOP).
0650213102 2012-07-20 Urban Drill / Deepen / Reopen No Violations On location to witness pit construction. The bedrock was encountered at 3-4 feet below the topsoil. I had Poulson Drilling install road fabric in the bottoms of the working and reserve pits prior to installation of the pit liner to prevent puncturing of the liner. No violations.
0691376746 2012-08-03 Urban Drill / Deepen / Reopen No Violations On location to check on closing the drilling pits. The cuttings have been properly solidified and buried in the liner. No violations.
0911981527 2012-07-21 Urban Drill / Deepen / Reopen No Violations On location at 1:00 A.M. to witness cementing of the 8.625 surface casing.
1254331198 2013-05-16 UrbanProduction Wells No Violations I was on location to check if a gate had been installed on the lease road. Steel poles had been installed on each side of the road, but the gates had not been put in. I called Alan Lorinchack (Summit Petroleum) and asked to get the gates installed within a week. He said he would take care of it. I will check back next week. No violations.
1343399592 2012-07-25 Urban Drill / Deepen / Reopen No Violations On location to check the drilling progress and inspect the pits. The Poulson rig is drilling at 5ft/hr above the top of the Newburg at 3848 at the kelly bushing (KB). The pits and liners are in good condition and have plenty of freeboard. No violations.
1345161235 2012-10-18 UrbanPreliminary Restoration No Violations During a status check of this well I found that the posts for the lease road gate had been set off of State Route 82. Posts had been placed around the pumping unit and were awaiting the fencing to be placed on them. The area around the well has been reclaimed, seeded, and is beginning to fill in with grass and clover. The tank battery for this well is located at the Craver-Nedalka #1. No violations.
1351789554 2012-09-12 Urban Drill / Deepen / Reopen No Violations I performed this status check inspection to check the progress of the installation of the surface equipment following completion of the well the previous week. I found a pumping unit on location and it had not yet been spotted over the wellhead. A trailer with used rods and tubing was on location waiting to be run into the hole. Four frac tanks and one flow back tank were still present on the location. The lease road was in good condition. No violations.
1353005989 2012-11-06 UrbanPreliminary Restoration No Violations I was on location to check the preliminary restoration status of this well location. The site has been graded and seeded. The pumping unit is fenced and screened and the gate is locked. All drilling and completion equipment has been removed. The lease road is in good condition. Preliminary restoration is passed. No violations.
1541910152 2012-07-30 Urban Drill / Deepen / Reopen No Violations All drilling equipment has been removed from location. The pits are fenced and awaiting removal of the drilling fluids.
1922615492 2012-07-27 Urban Drill / Deepen / Reopen No Violations On location to witness cementing of the 4.5 production casing. Poulson Drilling drilled the 7.875 hole to 4396 at the kelly bushing. They ran 4391 of new 4.5, 10.5 Lb/ft production casing and Petroset mixed and pumped 200 sacks of Class A cement. See cement report for more details.
