Inspection Data

Inspections Performed

Inspection ID Inspection Category Inspection Date Inspection Type Inspection Result Inspection Comments
2743910 Primary Facility 2018-06-21 Routine/Complete Inspection No Violations Noted Waiting on cement truck to cement cellars.
2755221 Primary Facility 2018-07-24 Routine/Complete Inspection No Violations Noted Weather was 74 degrees F. and raining. It had been raining for a few days prior to inspection. Met with Rockdale Marcellus representative Jimmy Larsen on location. Rig was in the process of waiting on cement after the cement surface casing job. At the end of the job when the plug bumped the casing jumped out of the slips and then settled back to its original position. When the cement company released the pressure to check the floats it was determined that either the casing had parted or the floats were not holding. They then closed in the casing and held 193 psi on the casing. They intend on running a bond log at approximately 7pm. They are waiting approximately 12 hours from plug bump to ensure cement integrity. Reviewed the cement job log and no discrepancies were noted at time of inspection. Performed a walk around inspection of location. No violations noted at time of inspection.
2760876 Primary Facility 2018-08-06 Routine/Complete Inspection No Violations Noted Weather was 72° and sunny at time of inspection. Met with Rockdale Marcellus representative Jimmy Larsen at time of inspection. Well was a spud conductor. Well was covered with a rig mat at time of inspection. No violations noted at time of inspection.
2771612 Primary Facility 2018-09-06 Routine/Complete Inspection No Violations Noted Weather was 78° and sunny at time of inspection. Met with Rockdale Marcellus representative Danny Brooks on location. Rig had drilled the well to TD on 9/5/18. Rig was in the process of circulating 2 bottoms up before the cement job. Halliburton was on location to complete the cement job for the production casing. Reviewed the cement job program and no discrepancies were noted. When this well is complete the rig will move over to the 4H. No violations were noted at time of inspection.
2776844 Primary Facility 2018-09-19 Routine/Complete Inspection No Violations Noted Weather was 70° and cloudy at time of inspection. There were no Rockdale Marsellus representatives on location at time of inspection. Well had recently been drilled and cemented to TD. Well has a nightcap installed. There was a 0-5k psi analog gauge attached to the top of the nightcap that read 0 psi at time of inspection. The mouse hole had a steel plate sitting on it. No violations noted at time of inspection.
2790871 Primary Facility 2018-10-17 Routine/Complete Inspection Pending I met Luke Chesner and Marc McCartor of Rockdale when I arrived. The frac was temporarily on hold because there was not yet enough water in the MASS to begin the next stages. The area around the MASS looked good overall, with all puddles on the ground surface showing low conductance (.11-.56 mS/ cm). One of the large pump containments had several inches of water in it that showed slightly elevated conductance (2.56 mS/cm). Walking around the sitewide containment, the liner and berms appeared to be in good condition, and most puddles were low in conductance (.4-.83 mS/cm). Large (approximately 12" diameter) collapsible hoses were run from the MASS onto the sitewide containment, and one of these hoses was compressing a foam berm. When I informed Luke, he told me he would have the containment vacced and prop up the 12" water line over the berm. A vac truck was working on the sitewide containment caddy corner from the entrance; immediately outside of this containment I discovered an approximately 15-20 ga

2803376 Primary Facility 2018-11-15 Administrative/File Review No Violations Noted Results were received for sample ID #0939077 taken on 10/17/2018 (see pictures). Sample was taken from a 15-20gallon puddle located outside of containment. Elevated barium, strontium, and chlorides indicate that the fluid on the pad was wastewater; the result of the administrative inspection is Violations Noted.
2825015 Primary Facility 2019-01-09 Routine/Complete Inspection No Violations Noted Light activity on the site when I arrived. 2 temporary sand separators currently hooked up to the recently fraced wells. The onsite modular storage structure was still present, but a worker was pumping water from the tank to a bank of Baker tanks in containment. The area around the MASS was dry, and a downgradient sediment trap nearby showed low conductance (.13 mS/cm). A truck loading area at the Baker tanks had a 10' x 5' (1-2" deep) in front of it that showed slightly elevated conductance; I recommend that the puddle be vacced. The area where I noted fluid with elevated conductance on my 10/17/18 inspection had recently had fresh gravel placed on it.
2853446 Primary Facility 2019-03-13 Routine/Complete Inspection No Violations Noted Routine inspection conducted to assess overall site conditions. Two wells are still in flowback. Batteries of frac tanks are present on the pad within secondary containment. The containment appears to be in good condition overall. An aboveground storage tank is present on site. Several puddles on the pad to the south of the AST had a conductivity of between 1.1 and 1.6 mS/cm. Three GPUs are present n site, are labeled, and appear to be in good condition. Two steel double-walled produced water tanks are present, labeled, and appear to be in good condition.

No violations noted.
