| Routine inspection conducted to assess overall site conditions. One frac tank battery is on location within secondary containment. No problems with the containment were noted.

Two steel produced water tanks are present and in good condition. These sit within steel containment with spay-in liner in good condition. Water within this containment was less than 200 uS/cm. Three GPUs are present within containment. The water within the containment was unfrozen and had a conductivity of less than 100 uS/cm.

Two sediment traps appear to be built as permitted and functioning as designed. Compost filter sock is deployed as the E&S plan calls for. Site is stable. No E&S issues noted.

No violations noted.

2715784 Primary Facility 2018-03-29 Drilling/Alteration No Violations Noted Arrived at 14:15 with Ulyana Trynovich (OGI). Weather: foggy. H&P 513 was at 6987' on the East Unit 4H. Met with company man Brendon Jesson. BOPS tested on 3/25/18. Checked accumulator pressures and reviews IC cement job report for the East 4H. No violations were noted at the time of this inspection.
2734614 Primary Facility 2018-05-25 Drilling/Alteration No Violations Noted Three wells drilled on location. A frack is being staged at this time. All containment appears to be sound. No violations noted.
2747297 Primary Facility 2018-06-29 Routine/Complete Inspection No Violations Noted Three wells in the production phase, but remain in flowback. Three separators with no leaks detected. Baker tanks on secondary containment. Tanks are holding fresh water and are being used for frac operations at nearby.
2813363 Primary Facility 2018-12-06 Routine/Complete Inspection No Violations Noted Routine inspection conducted to assess overall site conditions.
