| On July 25, 2013, I conducted an inspection at CONSOLï¾’s Nv-55 site.  At inspection, CONSOL was into the lateral on the 55F well and was at around 11,000 feet.

Both the E&S plan and the PPC plan were in the mailbox at the site entrance.  The company needs to update the DEP contacts in the PPC plan.  I also found permits found permits for the A, B, C and F wells; I did not find a permit for the NV-55M well.

The company man exsplains this occured 12/22/2014 about 2:00 am in morning.It appears the regulator (Big Joe )over pressure or fatigue causing the source of gas.The safety equipment work as designed although serious damage occured. The seperation unit has been completely dismantle to be removed off site. the ignition soure is unknown at present time .78.73. General provision for well construction and operation.
