Inspection Data

INSPECTION ID DATE DESCRIPTION COMMENT VIOLATION ID
1818517 2009-08-06 No Violations Noted Well-site located, not constructed. Inspection performed as a result of a waiver review due to the proximity of the well-site to an Unnamed Tributary to Allegheny River. The E&S plan as compiled and drawn should minimize accelerated erosion and sedimentation, provided that the E&S plan is properly implemented and maintained.
1845462 2009-11-19 No Violations Noted pre-site meeting with operator
1849967 2009-12-10 No Violations Noted At the time of the inspection, the site has not been drilled.

Inspection performed to discuss E&S plan changes with the operator. Randy Morris and Jarrod Shultz were present for the meeting. The original plan called for the placement of culvert pipe in the stream. Due to restrictions from the Fish and Boat commission, it was decided that the operator would change the plan to avoid the stream. The stream will be protected with a line of silt fence backed with haybales. In addition, the location of the rig and the pit have been moved. The changes to the E&S plan, as discussed, will be submitted to the Pittsburgh office for approval.
