Inspection Data

INSPECTION ID DATE DESCRIPTION COMMENT VIOLATION ID
1777539 2009-03-17 Violation(s) Noted Well tag in place. Well is equipped for production. Access road stable.

Well site is stabilized with vegetation. No signs of accelerated erosion on road or site.

  • Inspection revealed the following deficiencies:

1) Discharge of industrial waste to the ground. - An oily discharge can be seen around the perimeter of the tank. There are several areas

in the soil around the tank that are dark or stained in color, and has a petroleum smell to it. There are also small puddles of an oily
substance around the tank, as well as on the sides of the tank and around the inspection portal. There is also a possible discharge from
the well head, as there is an area of dead vegetation starting at the well head and extending down-slope 10-15 feet.
    • The silt fence should be removed from this site now that the vegetation has taken hold, and the site is stabilized

1876728 2010-04-06 No Violations Noted -Inspection conducted as follow-up to Curtis Small's 3/17/2009 inspection noting Clean Streams Law violations.

-Site has been stabilized with vegetation and silt fence removed. -It appears that all noted leaks and spills from Mr. Small's inspection have been cleaned up. -The area of dead vegetation extending from the well head itself is also no longer visible. Vegetation has grown back. Overall the entire site has about 80% uniform vegetation.
