The surface casing was equipped with a makeshift head that the tubing came up through. The pumping unit was equipped with an electric motor and the motor was running at the time of inspection but the arms on the pumping unit were disconnected and was inoperable. The motor was tied off to a tree with tie down ratchet straps. There was contact information at the tank battery but there was no well number. The production tanks were only 2 feet apart. There is a leak at the valve on the load line near the bottom of the production tank inside the dike. The drain of the dike was open and the wall of the dike had been flattened in a places. There was two 55 gallon drums on site as well as production lines not in use. The production lines that were in use were laid on top of the ground. All producing leases shall be legibly identified in a conspicuous place on or near the well-head or the storage tank as to owner, lease name, well number, permit number where available, county, and an emergency telephone number. If multiple wells are being produced into the common tank each well-head shall be identified as to owner, permit number where available, well number and lease name. All oily materials in in dike shall be removed and disposed of properly. Dike drain shall be equipped with a functioning valve or stopper to prevent unintentional draining of contaminated water. Dike wall shall be repaired or rebuilt. Tanks shall be placed a minimum of 3 feet apart. Any production lines that are in an area expected to be under cultivation shall be buried. 9-9-05(A)10

-801727449 2010-08-09 Inspection this day with Mark Hines, new owner, found well with jack, not producing. Says they were calling this #26 and should show as PR. These wells are all mixed up with permit nos. Mark wants to get them correctly identified and producing but problem with legal ownership. Good ID's say Mark is owner, correct phone nos. This permit no plotting closest to well, may not be correct.
