-990793597 2011-09-22 Dike GPS +/- 14' N39.49936 / W082.10515. Dike has oil and water in it. Tank has been removed. Well is disconnected from production equipment and production line. Well equipped with 4.5" casing with 4.5" by 2" swedge 2" valve (closed) 2" bull plug with tapped for 1/4" (open). Located 3 feet from well is a drip and 1" steel lines, gas regulator, meter. Took photos.
1075394029 2004-01-20 The well was found incapable of production and shut-in. The well was equipped with the following: 4 1/2 inch casing, 2-4 1/2 inch swage, a valve and bull plug. Neither casing head or other casing strings were visible at the surface. Next to the well was a gas meter for domestic use. Adjacent to the well was a 100 bbl tank. Around the well site, various fittings, including a casing head and drip, were scattered throughout the weeds. (see photographs)

1264782125 2010-01-28 Inspection this day found well still idle, overgrown with weeds - house meter still at well but valve shut on casing and 2 x 4" swedge also shut. Found tank has been removed, dike area filled with fresh water and oil - appears tank had been drained in the dike area and then removed, possibly a year or more ago. Dike area approx. 75' x 10', about foot deep - area has all been covered with oil - oil has hardened into layer, some floating on water, some laying on rock bottom of dike. At one corner of dike where lowest, evidence of oil on the ground below the dike, suggesting dike area ran over, either when initially drained or during a rain storm afterward. Contaminated area needs removed with equipment, loaded into truck and hauled away, well needs plugged and land reclaimed. Photos. Consent agreement has not been met.

Clean-up needs to begin on oily contamination - will call supervisor and see what he wants done or if action has been started. I was instructed by supervisor to not write new NOV for this well as company already in non-compliance. Currently hunting for operator to start clean-up, supervisor says.

841687175 2010-06-08 Met this day with Supervisor Stonerock and Roger Barr, out on service rig in Guernsey Co. When asked what happened to put oil in the pit, Roger said Troy McMannaway had drained oil and taken tank, he thought a couple years ago. Explained to Roger the charges against him and asked him to write letter to ODNR stating his intentions with the remaining wells and committing to when he could get them plugged. Said he had to finish well he was on then hoped to get to them shortly. Supervisor expressed to him the importance of quickly getting work done and need to put his intentions in writing. Said he would contact his attorney and take care of it.
868458782 2009-08-12 Inspection due to request from Supervisor - found could not get to well today - too overgrown. Talked to landowner mowing and he said well still there, nothing has been done to it. No evidence that ground or trees have been disturbed so believe he is correct - last inspection was 2007 and he has owned ground longer than that, he says.
