1867780 2010-03-05 No Violations Noted Drilling appeared to have been completed at the time of the inspection. Below grade pit located onsite. The pit had been relined and was being filled with freshwater, most likely for the purpose of well stimulation. The majority of the erosion control measures implemented on the access road or well-site were not able to be evaluated due to the large amount of snow still located onsite. The operator should continue to monitor the site and access road erosion controls to insure that they continue to function properly.
1873776 2010-03-31 No Violations Noted Drilling appeared to have been completed at the time of the inspection. A pipe was observed attached to the well head. This pipe emptied into the below grade pit located onsite. Noted that the silt fence had been installed correctly. No accelerated erosion & sedimentation was observed in relation to the access road or well-site. The operator should continue to monitor the erosion controls to insure they continue to minimize accelerated E&S until site restoration activities are completed.
1885581 2010-05-18 No Violations Noted At the time of inspection, the well was drilled and in production. Site was properly signed with E&S plan available for review. Access road was stable, disturbed areas were seeded and mulched surrounding the access road, as well as the well site, some growth was observed (not yet >70%). No accelerated erosion or sedimentation issues were observed in relation to well site or access road. Noted that silt fence was reinstalled down gradient of well pad and properly anchored into the ground. Recommend operator continue to monitor all erosion control measures until final restoration activities have been completed and the site has been permanently stabilized. (Accompanied with, Water Quality Specialist, Greg Higgins during inspection).
1955584 2011-03-08 No Violations Noted At the time of inspection, well tag present. Well in production. Drilling completed on 02/25/10. Site appears restored, although areas along access road (near entrance to pad) may need reseeded due to plowing activities. Observed that sides of brine tank were discolored, (red, rusty color) possible evidence of a prior release or over flow. Recommend that operator ensure the structural integrity of brine tank is not compromised and that tank is not full, dept unable to determine level of contents in tank during inspection. Brine tank located on shale base. No leak or active discharge detected although minor Es concerns noted behind brine, area may also need to be reseed or additional rock (as it appears rock may have been previously utilized around tank and production equipment.) be added to ensure ALL areas of site exhibit are permanently restored and able to resist the potential for erosion. Temporary bmp﾿s currently installed. No accelerated (offsite) ES issues noted.
2127020 2012-12-21 No Violations Noted At time of inspection, well in production. Well tag present. Access road adequately rocked. Noted that sides of brine tank were a red, rusty color which could be evidence of a prior release or overflow. No active discharge or leak observed. Same issue noted in prior inspection performed by WQS April Weiland on 3/8/11 (ID 1955584). Department unable to determine content levels during inspection.

Department requests that Operator conducts inspection of brine tank to ensure that structural integrity of tank is not compromised in order to prevent a possible pollution event from occurring. Department requests that Operator notify Samantha Foulk of the results of tank inspection and any actions taken in response to this request. UPDATE: 1/14/13 Received written response from Operator who stated that actions were taken to ensure integrity of brine tank was not compromised and no active leaks were detected. (See attached).


2142668 2013-02-21 No Violations Noted At time of inspection, well in production. Well tag present, legible. Access road in excellent condition. No accelerated E&S issues observed, although site snow-covered. Brine tank sound. Red staining on BT still prominent; however, concerns addressed in previous inspection (ID 2127020) and subsequently resolved.

Department recommends Operator continue to monitor and maintain access road and well site as necessary.
