This well appears equipped for production. Drill rig was on site for well 2H. No leaks of production fluid were evident. Incipient erosion channel was noted off southern end of pad inside LOD near stake for pipeline. Fabric was drooping on silt fence/super silt fence in two areas and should be mended. This site has a restoration extension until November 9. 2015.

2504529 Primary Facility 2016-04-07 Routine/Partial Inspection No Violations Noted On 4/7/16, I performed a routine partial site inspection at the Dorothy Bovard site in response to a request from the operator and met with Range Resources representative Erica Taylor to review requested alteration to the shape of the pad post restoration. The change was approved as a redline change as it did not appear to significantly change flow around the site and decreased impervious surface area slightly.
2566363 Primary Facility 2016-11-16 Complaint Inspection No Violations Noted On 11/16/16, I performed a complaint inspection related to the Bovard site and made the following observations:

•The complainant reported that beginning in late September 2016 their well began producing gray water. Within a few days the water changed to black with intermittent gray at which time the complainant called their well service company to inspect with inconclusive results. At this time the floss filter was reported to have begun clogging regularly and a rotten egg odor was noted. •The complainant reported being in residence for 46-48 years. The well bore is located roughly 10 feet from the house and had a secure cap elevated 6” off of the ground at the time of inspection. The complainant reported that the well was drilled to 240 feet deep in 1988 and has a poly case to 220 feet. The well was reported to have had prior issues with sediment. •Samples were taken from the well for 946 and bacteriological analysis before treatment and after a ten-minute flush. •Inspection of the well site found no i

2571026 Primary Facility 2017-01-13 Administrative/File Review No Violations Noted On 1/13/17, I performed an Admin inspection related to the Bovard Dorothy site in response to complaint 323595 and made the following observations:

•Review of lab results for samples taken 11/16/17 as well as other situational information and found that, while water chemistry parameters were high in multiple instances, oil and gas impact was not conclusively indicated.

2626789 Primary Facility 2017-03-27 Complaint Inspection No Violations Noted On 3/27/2017 a COMPL inspection of the Bovard well was completed in response to complaint 325615. The following was observed:

>E&S controls in multiple locations around the site were observed to be unnecessary and/or in poor repair. All controls should be removed or maintained as appropriate.

>No evidence of spill or other surface contamination was noted in relation to possible water supply impact.
