2803933 Primary Facility 2018-11-15 Routine/Complete Inspection No Violations Noted This location can most easily be accessed off of Mohawk Avenue in Pleasant Township, Warren County, Pa.

The location was recently constructed. The new location and access road have been stabilized with a combination of local and commercial stone. Some vegetation has been established on the banks and in some of the ditch-lines. All relevant E & S related BMP's seem to be acting effectively. The drilling permit is clearly posted on location. An E & S plan is also posted near the location. The container that the plan has been placed in is apparently leaking. The plan has become very wet and therefore was not reviewed. The operator is in the process of drilling the well. They are currently preparing to run cement in an attempt to plug off voided space that was noticed at around 200'. Drill cuttings have been directed into a pit. Portions of the pit have been lined to protect against erosion. Excess water is being pumped from the pit and broadcast in varying directions.
