The E&S Inspection Log indicated that Dillon Otten (Tioga Environmental Consulting) conducted an inspection on 2/28/13. It was noted that silt sock has been red lined at the entrance. This silt sock does not appear to be installed efficiently. Operator, please review the placement of this silt sock and make any appropriate adjustments. The area across from the tank pad entrance does not appear to be temporarily stabilized. Please note that 25 PA Code §102.22(b)(1) states “Upon temporary cessation of an earth disturbance activity or any stage or phase of an activity where a cessation of earth disturbance activities will exceed 4 days, the site shall be immediately seeded, mulched, or otherwise protected from accelerated erosion and sedimentation pending future earth disturbance activities.”Overall the site construction, along with red lined additions, appears to be installed as designed. |

The operator reported that they discovered the release on 4/6/14 and that they believe that during the night of 4/5/14 an act of vandalism occurred. The operator stated that the vandal stole diesel from Williams construction vehicles that were staged on site. Cabot explained that upon discovery, they began to remove the material from the site. The material was staged on site in a 30 yard roll-off. Resource Environmental was contracted to handle the remediation. Frank Hoehle and I arrived on site and noted two excavations and the lined roll off container. The excavations were approximately 4’X4’. |
