Inspection Data

Inspections Performed

Inspection ID Inspection Category Inspection Date Inspection Type Inspection Result Inspection Comments
1720560 Primary Facility 06/18/2008 Drilling/Alteration No Violations Noted site pit e/s ok
1773961 Primary Facility 03/04/2009 Routine/Complete Inspection No Violations Noted Drilling appeared to have been completed at the time of the inspection. Production equipment was located onsite. Included were two brine tanks and they appeared to be in sound condition. No accelerated erosion & sedimentation was found on either the access road or well-site. It was noted that there appeared to be freshwater seeping from the fill area below the above grade pit constructed onsite for stimulating the well. Additionaly it was noted that the pit was relatively empty. Contacted operator representative Gary Bolen concerning this matter and he stated that the pit in question only contained freshwater, and no flowback, and that he would check into the future of the pit, i.e. whether it can be regraded and if a secondary frac is neccesary build another above-grade pit.
1991483 Primary Facility 07/27/2011 Incident- Response to Accident or Event No Violations Noted
