0085641083 2018-07-12 Production Wells No Violations I did an Order follow-up inspection (Chiefs Order 2016-391) and a Compliance notice follow-up inspection for this well. The well is not plugged per the Compliance Notice and Chiefs Order 2016-391 for Failure to Maintain Liability Insurance has not been terminated. The well is equipped with a pumpjack and the well is being produced. The sign at the tank batter is an Everflow Eaststern sign. I contacted George Strawn (Everflow Eastern), he advised me Everflow Eastern is pumping the well. The tank battery consists of a steel production tank, a horizontal separator and a meter. The tank valve is locked, and bull plugged. The well gate is locked. No Annual Production Reports have been submitted since 2012. I left a message for Mike Schreiner (330-323-2277) for a return call.
1479220019 2016-11-15 UrbanProduction Wells No Violations The well is operated as an electric pumping well and surrounded with a locked stockade fencing. The I.D. signage reflected Eastern Everflow as Operator. The valves on the well head were in the open position. The stuffing box appeared to been recently greased. The electric panel was in the on position. Operator has not reported production volumes since 2012. The electric meter was still in place but not turning during this inspection. The gas meter has a chart hung, but without dates. It indicated that the well was set to run one (1) time a day. The tank battery consisted of one (1) 100 barrel oil tank, one (1) horizontal separator and gas meter. An earthen dike surrounded the tank battery. The load lines were locked and plugged. The tank battery was absent of contamination and leaks. The I.D. signage, at the tank battery, again reflected Eastern Everflow as the Operator. I placed a call to George Strawn ( Eastern Everflow ) to sort out the confusion. The Well Owner / Operator shall submit a written plugging schedule to Robert Roush at 3575 Forest Lake Drive, Suite 150, Uniontown, Ohio, 44685 within 30 days, per OAC 1501:9-11-05 (B), and plug the well within 180 days. The Well Owner will be notified if the plugging schedule is approved or denied.
