1823888 Primary Facility 2009-08-13 Drilling/Alteration No Violations Noted Site inspected with OGI Bruce Jankura. Also spoke with Drilling and Completion Consultant, Kelly Spoonts on site. Conductor pipe and top hole had already been drilled, drilling of horizontal section was in progress. All disturbed areas have been seeded and mulched with about 45% vegetation already grown back in. Site has brush row reinforced with line of silt fence surrounding parameter. All E&S control BMPs have been installed properly and are working adequately. The pit is constructed and lined properly. Liner appears sound with no visible cuts or punctures. More than 2ft of freeboard is being maintained
1824644 Primary Facility 2009-08-27 Complaint Inspection No Violations Noted Site was inspected from perimeter of pad. In response to complaint #265864, Alex branch was field tested from just below McGregor Rd Crossing to headwaters. Stream begins approximately 700 Ft from edge of well pad. Meter readings indicated elevated TDS and Conductivity here. Pit appears sound. A properly installed sediment fence reinforced with haybales follows the edge of the pad. The are no signs of erosion or that any fluids have left the site on the surface. Inspection was conducted jointly with the following personel from the PA Fish and Boat Commission: Waterways Conservation Officer, Robert V. Dunbar, Fisheries Biologists, Steven Kepler and Heather Smiles. Officer Dunbar spoke with Matt Williams on-site. Mr. Williams mentioned that the well had low water returns.
1829048 Primary Facility 2009-09-04 Routine/Partial Inspection Violation(s) Noted Miller - on-site meeting w/EOG & PaFBC reps, multiple investigations between 8/25/09 & 9/4/09 resulted in determination that activities at this well site released contaminants from Marcellus shale drilling to atmosphere, written response requested w/in 10 days. See 401CSL violation comment. NOV to MBC for input 9/21, insp not previously input. MBC 9/21/09
1829540 Primary Facility 2009-09-10 Follow-up Inspection No Violations Noted Site was inspected with Nathan Wells, Tom McNerney and Caleb Woolever. Pit had been excavated and all recovered materials placed in 9 containers that have been covered and stored on-site until approval has been aquired to haul them to landfill. A water truck comes every other day to pull any water produced from contents. Pit has been backfilled with cleanfill. Field testing shows TDS, conductivity and pH levels in stream originating about 700' from pad have improved significantly. Casing being run on day of inspection, no E&S problems at this time

1836100 Primary Facility 2009-10-14 Drilling/Alteration Violation(s) Noted Spill response: Monday, October 12, 2009 Joe Wilson, independent consultant, noticed spring behind 8H was cloudy and sudsy. EOG was notified and found that a gasket on one of the holding tanks had gone bad. approx. 190 bbls of fluids were unaccounted for. There was no indication of a spill on-site. Fluid containing about 98% fresh water, 1.06% well wash and .25% ultra vis, soaked into the pad and made its way to the nearby stream. The stream had booms placed in it and the water was being pumped out to be contained on site. Suds were seen as far down as Camp Read. EOG also flushed the area a couple of times with fresh water. They will continue to monitor the stream quality and flush if necessary. Greg Shaffer, of EOG along with Joe wilson were on site during the inspection. Representatives of the Fish and Boat Commission were also present

Production processing equipment on location includes 1 production water tank, 1 gas processing unit, measurement equipment ¿ orifice meter stations.
