The large pit had approximately 2' of freeboard during inspection. At the time of inspection, the crew was setting a 30' casing seal at 6156'. I was informed by the crew on Site that a leak was discovered at the port collar at a depth of 6171'. The section around the port collar was cemented and the leak was reportedly stopped. Once the seal is set, the well will be ready for fracking.
1803478 Primary Facility 2009-06-01 Follow-up Inspection Violation(s) Noted On May 31, 2009, a spill of 120 gallons of hydraulic fluid was reported at the site. An inspection was completed following a spill reported at the site on May 31, 2009. The spill occurred at approximately 04:00 when a line ruptured from a 120 gallon hydraulic fluid reservoir on one of the blending trucks. DEP Emergency Response conducted the initial inspection at the time of incident and filed a report.

The site was constructed according to plan, and there was active logging occurring across the access road to the Well Site. The logging activity was not causing issue with the site other than directly on the access road leading to the well site. The repairs to the site were all vegetated with perennials and performing to standard. The site was stable.

2622444 Primary Facility 2017-08-04 Routine/Complete Inspection No Violations Noted I inspected the site on July 24,2017 and found the small site to be very well vegetated and stable. The cut and fill slopes were healthy and had no issues at the time of inspection. The access road was stable and not muddy as well. There was no bubbling in the well cellars observed. The pipe insulation from the GPU to the 8h was deteriorating and should probably be cleaned up.

There were no violations at the time of inspections.
