Inspection Data

INSPECTION ID DATE DESCRIPTION COMMENT VIOLATION ID
1840726 2009-11-03 No Violations Noted Drill rig located onsite and drilling was ongoing at the time of the inspection. Drill pit was located onsite at the time of the inspection. E&S plan had been implemented as outlined, and no accelerated erosion & sedimentation was observed in relation to the access road or well-site. The Department does recommend thet the operator implement additional rock on the access road due to the lack of other BMP's to remove runoff from the access road. The rock would serve to min imize the amount of sediment transported off-site during well stimulation activities or moving the rig off-site.
1849576 2009-12-14 No Violations Noted Drilling had been completed at the time of the inspection. Drill rig not located onsite at the time of the inspection. Below grade pit located onsite at the time of the inspection and it had more than the required two feet of freeboard. Observed no accelerated erosion & sedimentation with respect to the access road or well-site. The operator should continue to monitor the site and access road to insure the erosion controls continue to minimize accelerated erosion & sedimentation unitl final site restoration activities are completed.
1876332 2010-04-06 No Violations Noted Drilling had been completed, well appeared to be plumbed and in production at the time of the inspection. Below grade pit had been filled in and that area had been regraded. The remainder of the site was not regraded and had not been seeded & mulched. The operator should regrade, seed, and mulch the site as soon as possible to restore the site within the growing season, provided that the well is not be stimulated. The operator is also reminded of the nine month due date for site restoration from the date of drilling completion.
2073724 2012-06-06 No Violations Noted Well appeared to be equipped for production. Site had been properly regraded and the vegetative cover met the 70% uniform coverage criteria. There was no observed accelerated erosion or sedimentation in relation to the access road or well site.
