0505619082 2017-04-12 Preliminary Restoration No Violations On the day of my inspection accompanied by Inspector Randy Akins I witnessed metal containment has ten 500 barrel tanks, seven are condensate and three are produced water. Production equipment including flares, compressors, heater treaters, and seperators on site. Well is equipped with christmas tree type welhead with open valves. Valves show 150 psi on surface casing annular and no pressure on intermediate annular.
1287772777 2015-04-02 Casing String Witnessed No Violations I am on site today to witness the running and the cementing of the 30 inch diameter casing string. Precision Rat hole service drilled a 36 inch diameter hole to 100 ft. and set 100 ft. of 118 lb. casing. Universal Well cemented with 310 sacks of class A cement at 15.6 lb. and a yield of 1.18 - had good cement returns to surface.
1435677025 2015-06-30 Casing String Witnessed No Violations I am on site today to witness the running and the cementing of the 13-3/8 inch diameter casing string. Patterson Drilling Rig # 284 drilled a 17.5 inch diameter hole to 1309 ft. and set 1312 ft. of 61 lb. casing. O-Tex Cementing Service cemented with 1350 sacks of class H cement at 15.6 lb. and a yield of 1.20 Had good cement returns to surface.
1436411074 2015-07-08 Casing String Witnessed No Violations I am on site tonight to witness the running and the cementing of the 9-5/8 inch diameter casing string. Patterson Drilling Rig # 284 drilled a 12-1/4 inch diameter hole to 7267 ft. and set 7265 ft. of 40 lb. casing. O-Tex Cementing Service cemented with 50 bbl. Of Barite at 11.5 lb. and 521 sacks of class H lead cement at 12 lb. with a yield of 2.15 and 281 sacks of class H tail cement at 15.3 lb. with a yield of 1.26 then displaced and dropped the opening tool for the DV devise set at 3918 ft. The tool opened and 56 bbl. of cement circulated out to surface. Circulation continued for 5 hours until stage two was started. 50 bbl. of Barite at 11.5 lb. was pumped followed with 1200 sacks of class H tail cement at 15.3 lb. with a yield of 1.26 Circulated good cement to surface.
