1141705939 2006-02-24 Jim Hershman complaint of idle well and occasional H2S gas odor . Inspection of the well revealed that the well is idle . The valves on the wellhead are all closed and rusted . Parts of the separator is missing and the gas sales line is unhooked . I am issuing a written NOV to Atlas America to plug or produce the well . Note: There is a faint H2S gas odor at the wellhead at this time . I didn't have my gas detector with me to check the H2S gas levels .
1145988249 2006-04-19 On site to witness the bottom hole plug ( Clinton Plug ) . Formation cementing pumped a 25 sk plug thru tubing to plug from 5644 to 5344 . H2S gas odor while they were pumping the plug . Gary Haught advised that they were going to rip the 4 1/2 casing and pull the casing because there is a hole in the casing . They are going to run tubing back into the well to finish plugging the well . Should be ready on Monday morning to finish plugging the well .
-1228800905 2006-04-18 Inspection revealed that Cardinal Well Services has fished the 1 1/2 tubing out of the well and are running the 2 3/8 upset tubing back into the well to spot the bottom hole plug tomorrow morning .
1500062950 2006-04-21 On site today to finish plugging the well . Pumped the Clinton plug on 04/19/2006 . Pumped the lime plug , bottom 8 5/8 plug , berea plug , and the surface plug . Well has plugged and abandoned . This well is associated with the Jim Hershman complaint . Case can be resolved at this time .
-1685794241 2013-09-16 On the day of my inspection I witnessed that the pit had been filled, vegetation was in place. All production equipment had been removed. Site passed according to 1509.072B.
1875421302 2006-04-18 Inspection revealed that Cardinal Well Services is running a fishing tool back into the well with tubing ; trying to fish out the 1 1/2 tubing that H2S gas rotted of at app. 2785'. They got ahold of the tubing with the fishing tool and started coming out of the hole . I advised that I would return to the site later today to check on the progress of the fishing job.
667582127 2006-04-13 Inspection revealed that the well is still idle . Talked with Terry Whitehair and he advised that they are applying for the plugging permit and that they need a couple of more weeks to get the well plugged . I advised that they can have an extension until 04/28/06 .
