While on site I received the cement information for the 5-1/2 inch diameter production casing which was blank when I checked my ticket. I will also fill out a Cement Ticket to update all information. Nomac Drilling Rig # 71 drilled a 9-5/8 hole to 15,451 ft. and set 15,451 ft of casing. Schlumberger Cementing Service c/w 870 sks of lead @13.5 lb with a yield of 1.67 and 1852 sks of tail cement @ 15 lb with a 1.3 yield. Set date was 08/14/2013 they indicated 4 barrels of cement returns.

| I arrived on location accompanied by inspector Mark Moreland, were I witnessed the lease road gate was locked and the lease road in adequate condition. The pad is fenced and all man doors were locked.  Wells with permit numbers 34067212200000 and 34067211080000 are operated by a plunger lift system. Permitted well number 3406721218000 is a reservoir pressure driven well. RBDMS production records indicate that the wells are in production.

While on site I received the cement information for the 5-1/2 inch diameter production casing which was blank when I checked my ticket. I will also fill out a Cement Ticket to update all information. Nomac Drilling Rig # 71 drilled a 9-5/8 hole to 15,451 ft. and set 15,451 ft of casing. Schlumberger Cementing Service c/w 870 sks of lead @13.5 lb with a yield of 1.67 and 1852 sks of tail cement @ 15 lb with a 1.3 yield. Set date was 08/14/2013 they indicated 4 barrels of cement returns.
0268029142 2013-08-15 Drill / Deepen / Reopen No Violations I arrived today to check Nomac Drilling Rig # 71 and found that they are moving today to the # Long 6H and should be ready to spud tonight and set surface Friday. I was also on site to collect cement information but while checking I discovered that the reports all had -0- in all of the boxes. I was advised that they would request the corrected information and send me a copy.
0715224034 2016-02-16 Preliminary Restoration No Violations I arrived on location accompanied by inspector Mark Moreland, were I witnessed the lease road gate was locked and the lease road in adequate condition. The pad is fenced and all man doors were locked. Wells with permit numbers 34067212200000 and 34067211080000 are operated by a plunger lift system. Permitted well number 3406721218000 is a reservoir pressure driven well. RBDMS production records indicate that the wells are in production.
The production equipment on the pad consists of the following:

7 400 bbl. Production tanks 3 Producing wells 3 Sand separators

1 Low pressure separator tower/Vapor recovery system unit. 1 Combustor 1 Gas compressor unit. 1 Horizontal heater-treater. There was approximately 4 to 5 inches of snow on location at the time of my inspection. The site passed preliminary restoration in accordance with ORC 1509.072 A.
