| An inspection was conducted with Oil & Gas Inspector, Brandon Trigg, in response to a severed drill pipe incident on the Nomac Rig #72 drilling for Chesapeake Energy.  The Division did not receive notification and this incident reportedly occurred at 0200 hours on 9-7-14. The rig was drilling in the Clinton and was going into the Queenston. The rig lost the Rate of Penetration reading or (ROP) and begin working  drill string up and down. There was a slight drag in the process. There was internal explosion in the drill string. It shot the pipe up and slammed the pipe back down causing a crack in the elevators for the 5''19lb/ft. drill pipe. The 5''19 lb. per foot drill pipe has a burst of 17,100 psi. The standpipe on the mast blew a 10,000 psi Demco valve apart. The Kelly Hose was blown apart and blew shrapnel everywhere. The top drive had internal parts that were wiped out and had to be replaced. There was a blue air tank rated for 150 psi that was hit with shrapnel and blew a hole in it. The company man named Matt Whitehead escorted Brandon Trigg and Patrick Shreve on the tour. Roland Reynolds was the company Man on duty at night and the Drilling Superintendent was Dave Newman. They are now on days off. There was no Notification to the Division staff on duty at the time of the incident. Whitehead told us that they have to go 400' fishing and there will be 5,000 feet of drill pipe to fish on. The pipe may be locked in by the shale swelling in the Queenston. The reason for the incident is under investigation by Chesapeake. The rock oil used to lubricate the hammer bit may have reacted with a airborne substance. There may have been a problem with an air compressor blowing by oil and a spark by the drag of the drill string and some Clinton gas and all 6- 1170 air compressors and the 2- boosters at the same time could have been a factor. The dry adds are stored over by the compressors and the boosters. There could have been some kind of oxidizer sucked into the breather of an air filter and ignited with the rock oil combined with all the air pushed down the drill string. There were no casualties and the hole is now mudded up. The day Brandon and Patrick were there they were pressure testing the Blow out Prevention equipment and the chokes. Pictures were taken of the equipment that was affected. Pictures were taken of the the new replacements.

| On the day of my inspection I found Nomac Rig #72 preparing to do a cement squeeze at the depth of 5968 feet,  the drill pipe was perforated at 5580 feet, with 5, 2 1/8 shots, at an interval of 8 inch spacing. Using 30 bbls mud push and 185 sacks of cement, displaced with 75 bbls of water leaving 3000 feet of cement in the drill pipe. All was shut in for a 3 hour period to avoid any flow back and that cement would stay as pumped. An 800 foot plug was set at 3880 to 3080 to seal off the top of the tools left in the hole and use as a kick off point around the lost tools. 391 sacks of cement was used for the top plug.

| On the date of inspection I accompanied ODNR Inspector Pat Shreve to interview the company man on duty, Matt Whitehead, about the incident that took place on the site and to photograph the location of equipment on site. The photographs are uploaded to the shared network drive. I was made aware of the incident while on a job at the Chesapeake Wright Pad. Chesapeake Drilling Superintendent John Snook was on location at the Wright Pad and was not on duty at the time of the incident. John Snook made me aware of the incident due to the fact that he had just visited the Jamar Pad prior to being on the Wright Pad location. Refer to Pat Shreve's inspection for further details regarding the incident.

| An inspection was conducted with Oil & Gas Inspector, Brandon Trigg, in response to a severed drill pipe incident on the Nomac Rig #72 drilling for Chesapeake Energy.  The Division did not receive notification and this incident reportedly occurred at 0200 hours on 9-7-14. The rig was drilling in the Clinton and was going into the Queenston. The rig lost the Rate of Penetration reading or (ROP) and begin working  drill string up and down. There was a slight drag in the process. There was internal explosion in the drill string. It shot the pipe up and slammed the pipe back down causing a crack in the elevators for the 519lb/ft. drill pipe. The 519 lb. per foot drill pipe has a burst of 17,100 psi. The standpipe on the mast blew a 10,000 psi Demco valve apart. The Kelly Hose was blown apart and blew shrapnel everywhere. The top drive had internal parts that were wiped out and had to be replaced. There was a blue air tank rated for 150 psi that was hit with shrapnel and blew a hole in it. The company man named Matt Whitehead escorted Brandon Trigg and Patrick Shreve on the tour. Roland Reynolds was the company Man on duty at night and the Drilling Superintendent was Dave Newman. They are now on days off. There was no Notification to the Division staff on duty at the time of the incident. Whitehead told us that they have to go 400 fishing and there will be 5,000 feet of drill pipe to fish on. The pipe may be locked in by the shale swelling in the Queenston. The reason for the incident is under investigation by Chesapeake. The rock oil used to lubricate the hammer bit may have reacted with a airborne substance. There may have been a problem with an air compressor blowing by oil and a spark by the drag of the drill string and some Clinton gas and all 6- 1170 air compressors and the 2- boosters at the same time could have been a factor. | On the day of my inspection I found Nomac Rig #72 preparing to do a cement squeeze at the depth of 5968 feet, the drill pipe was perforated at 5580 feet, with 5, 2 1/8 shots, at an interval of 8 inch spacing. Using 30 bbls mud push and 185 sacks of cement, displaced with 75 bbls of water leaving 3000 feet of cement in the drill pipe. All was shut in for a 3 hour period to avoid any flow back and that cement would stay as pumped. An 800 foot plug was set at 3880 to 3080 to seal off the top of the tools left in the hole and use as a kick off point around the lost tools. 391 sacks of cement was used for the top plug.

| On the day of my inspection I found this pad containing 9 properly labeled wells that are producing. This well has a plunger lift system wellhead. Pressure gauges read as follows: Surface 18 psi, Intermediate 226 psi, Production 477 psi, and Tubing 222 psi. Casing hanger is plumbed into buried flow line. Intermeditate is also plumbed into flowline with a motor valve. Production equipment includes twelve 400 barrel steel tanks within their own steel containment, three compressors, two line heaters nine sand separators, nine GPUs, a vapor recovery stack, and a combustor. One heater treater onsite. Site is stable and vegetated. Site meets preliminary restoration requirements of 1509.072A.

The photographs are uploaded to the shared network drive. I was made aware of the incident while on a job at the Chesapeake Wright Pad. Chesapeake Drilling Superintendent John Snook was on location at the Wright Pad and was not on duty at the time of the incident. John Snook made me aware of the incident due to the fact that he had just visited the Jamar Pad prior to being on the Wright Pad location. 1024382522 2014-09-16 Casing String Witnessed No Violations On the day of my inspection I found Nomac Rig prepairing to do a casing test and FIT test. A test of 20 minutes at 508 psi, with a 2 lb drop.
1409814229 2014-09-04 Casing String Witnessed No Violations On the day of my inspection I found Nomad Rig 72 preparing to cement surface casing. 450 feet of 13 3/8 casing was installed into the wellbore. Schlumberger will do the cementing. Circulation was established and 33 bbls of good cement returns was circulated to surface.
1410522654 2014-09-11 Drill / Deepen / Reopen No Violations An inspection was conducted with Oil & Gas Inspector, Brandon Trigg, in response to a severed drill pipe incident on the Nomac Rig #72 drilling for Chesapeake Energy. The Division did not receive notification and this incident reportedly occurred at 0200 hours on 9-7-14. The rig was drilling in the Clinton and was going into the Queenston. The rig lost the Rate of Penetration reading or (ROP) and begin working drill string up and down. There was a slight drag in the process. There was internal explosion in the drill string. It shot the pipe up and slammed the pipe back down causing a crack in the elevators for the 519lb/ft. drill pipe. The 519 lb. per foot drill pipe has a burst of 17,100 psi. The standpipe on the mast blew a 10,000 psi Demco valve apart. The Kelly Hose was blown apart and blew shrapnel everywhere. The top drive had internal parts that were wiped out and had to be replaced. There was a blue air tank rated for 150 psi that was hit with shrapnel and blew a hole in it. The company man named Matt Whitehead escorted Brandon Trigg and Patrick Shreve on the tour. Roland Reynolds was the company Man on duty at night and the Drilling Superintendent was Dave Newman. They are now on days off. There was no Notification to the Division staff on duty at the time of the incident. Whitehead told us that they have to go 400 fishing and there will be 5,000 feet of drill pipe to fish on. The pipe may be locked in by the shale swelling in the Queenston. The reason for the incident is under investigation by Chesapeake. The rock oil used to lubricate the hammer bit may have reacted with a airborne substance. There may have been a problem with an air compressor blowing by oil and a spark by the drag of the drill string and some Clinton gas and all 6- 1170 air compressors and the 2- boosters at the same time could have been a factor. The dry adds are stored over by the compressors and the boosters. There could have been some kind of oxidizer sucked into the breather of a
1410851773 2014-09-16 Plug / Plug Back No Violations On the day of my inspection I found Nomac Rig #72 preparing to do a cement squeeze at the depth of 5968 feet, the drill pipe was perforated at 5580 feet, with 5, 2 1/8 shots, at an interval of 8 inch spacing. Using 30 bbls mud push and 185 sacks of cement, displaced with 75 bbls of water leaving 3000 feet of cement in the drill pipe. All was shut in for a 3 hour period to avoid any flow back and that cement would stay as pumped. An 800 foot plug was set at 3880 to 3080 to seal off the top of the tools left in the hole and use as a kick off point around the lost tools. 391 sacks of cement was used for the top plug.
1411652605 2014-09-24 Drill / Deepen / Reopen No Violations On the day of my inspection I found Nomac Rig 72 preparing to trip out of the hole, and run back in the wellbore with directional tools, they are ready to build there curve at 6610 feet.
1530202854 2018-04-10 Preliminary Restoration No Violations On the day of my inspection I found this pad containing 9 properly labeled wells that are producing. This well has a plunger lift system wellhead. Pressure gauges read as follows: Surface 18 psi, Intermediate 226 psi, Production 477 psi, and Tubing 222 psi. Casing hanger is plumbed into buried flow line. Intermeditate is also plumbed into flowline with a motor valve. Production equipment includes twelve 400 barrel steel tanks within their own steel containment, three compressors, two line heaters nine sand separators, nine GPUs, a vapor recovery stack, and a combustor. One heater treater onsite. Site is stable and vegetated. Site meets preliminary restoration requirements of 1509.072A.
1862986445 2014-09-11 Drill / Deepen / Reopen No Violations On the date of inspection I accompanied ODNR Inspector Pat Shreve to interview the company man on duty, Matt Whitehead, about the incident that took place on the site and to photograph the location of equipment on site. The photographs are uploaded to the shared network drive. I was made aware of the incident while on a job at the Chesapeake Wright Pad. Chesapeake Drilling Superintendent John Snook was on location at the Wright Pad and was not on duty at the time of the incident. John Snook made me aware of the incident due to the fact that he had just visited the Jamar Pad prior to being on the Wright Pad location. Refer to Pat Shreves inspection for further details regarding the incident.
2139144683 2016-02-23 Preliminary Restoration No Violations I arrived on location accompanied by inspector Mark Moreland, were I witnessed the lease road gate was locked and the lease road in adequate condition. The pad is fenced and all man doors were locked. The permitted wells 34067213530000, 34067213520000, 34067213510000, 3406721350, 34067213550000 and 34067213540000 are utilizing a plunger lift system. RBDMS production records indicate that the wells are in production. Permitted wells 34067213480000, 34067213490000 and 34067213470000 have not been drilled, but the tin horns have been set.
The production equipment on the pad consists of the following:

12 400 bbl. Production tanks 6 Producing wells 6 Sand separators

2 Horizontal heater-treaters 1 combustor 3 compressor 1 Low pressure separator tower/Vapor recovery system unit. 2 NGL tanks

The site passed preliminary restoration in accordance with ORC 1509.072 A.
