INSPECTION ID DATE DESCRIPTION COMMENT VIOLATION ID VIOLATION CODE VIOLATION_COMMENT
-1134131424 2010-01-12 The required 5 year MIT is still on hold at the request of the City. They are waiting for the Moore Well service rig to pull the tubing and set a packer to conduct the test. On this date they were producing some brine which was being discharged into their containment pit.
-1192659126 2009-02-02 Status check inspection found no violations at this time. The injection pressure will be checked at a later date. The ID is posted on the perimeter fence and the system was placing brine in the holding pit. The five year MIT is due this year.
-1544334726 2011-12-28 The City of Cuyahoga Falls continues to use this well to generate brine for use in the adjacent water treatment plant and for snow and ice control. During this inspection, brine was being pumped into the holding pond and City trucks were loading for road application.
-2071468511 2004-02-06 Status check inspection of solution project prior to meeting with John Christman regarding MIT test. No violations at this time.
-851761336 2010-03-22 Moore Well Service is on site to initiate the five year MIT requirement. The 2-3/8" tubing has been removed and the plan is to run a scraper in the morning before attempting to run the packer necessary for the MIT.
1081340000 2004-04-06 Completed the Mechanical Integrity Test on the 5-1/2" casing in this solution mining well. Ran a Halliburton RTTS retrievable packer on 2-3/8" tubing. Set packer at 2663'. Pressure up to 150# and found a leak at the tubing head. Tighten and retest to 500#. Had a small leak in the packing of one the old valves. The test had to be done through the old valves because of the connection sizes. This leak probably accounted for the 3% pressure loss (500 psi to 485 psi) after the 15 minute test. All of the valves will be replaced before the system is placed back into operation.

Initial - 500 psi 5 Minute - 500 psi 10 Minute - 495 psi 15 Minute - 485 psi TEST PASSED.

1269266382 2010-03-19 The well is scheduled for a 5 year mechanical integrity test. Moore Well Service is bringing a service rig on Monday 3/22 to start the project which will include removing the tubing, running a retrievable packer and completing the MIT.
1270502005 2010-03-23 Met on site with Keith Moore representing Moore Well Service and discussed the planned activity. The MIT is scheduled for 3/24.
1739716426 2004-02-04 Status check inspection prior to conducting MIT.
253529591 2004-04-08 Run 2-3/8" tubing back into the hole, pressure up and start circulating.
621302873 2010-03-24 on site for MIT test of well casing 51/2". Ran a Baker #81 Packer on 23/8" tubing set at 2680'.ran pressure up to 380PSI shut in waited 10 minutes no leaks. Then bleed off pressure then ran MIT test pressure up to 525PSI shut in started time at 5min press
