Well operating and properly identified. Well producing off 7" x 2" swage and equipped with meter. 50 psig on wellhead at time of inspection. Well Record and Completion Report shows 1350' of 20 lb/ft 7" "limited service" casing in hole with a cement basket set at 127' with recorded cement returns on surface casing. Operator needs to comply with Chapter 78, including but not limited to, use of safety devices - well casing, use of safety devices - blow-out prevention equipment, and casing standards. Met w/Operator rep Aaron Klingensmith. Well site reclaimed and vegetated.

2208319 2013-09-17 No Violations Noted Well shut in on 7ï¾” x 2ï¾” swage. 15 psig on wellhead at time of inspection. At surface, zero percent (0%) of gas measured (MSA 62S combustible gas indicator) outside 9 5/8ï¾” conductor casing and within 9 5/8ï¾” x 7ï¾” annulus. Pit lined and freeboard maintained. Inspection conducted w/WQS G. Higgins.

Per Operator rep S. Baker well deepened from 1468ï¾’ TD to 3200ï¾’ TD Driller with drilling start date of 09-09-13 21:00 and drilling completed date of 09-10-13.

2226686 2013-12-05 No Violations Noted Drilling appeared to have been completed. Site and access road controls had been implemented properly and there was no observed evidence of accelerated erosion or sedimentation in relation to the site or access road. Drill pit located onsite and it contained fluids and drill cuttings. The drill pit had more than the required two feet of lined freeboard.
