| Drilling was not ongoing, drillers were waiting to cement the string of casing that they had just finished placing.  Pit had been lined and had more than the required two feet of lined freeboard.  It did appear that nothing had been done to address the construction issues related to the pit.  This could not be confirmed due to the pit being in use.  Observed no additional accelerated erosion onsite.  Operator had sparsely mulched the fill slope disturbed area.  For future site construction the operator should endeavor to have the pit sidewalls sloped instead of perpendicular to the bottom of the pit and silt fencing should be installed so as not to concentrate the runoff flow to one area of the silt fence which will inevitably result in the he failure of the silt fencing.

| Drilling was completed on 06/10/11, well appeared to be equipped for production.  Brine tank located onsite and it appeared to be in sound condition.  Vegetation had grown in thicker, however the erosion issues had not been addressed as of the date of the inspection.  Specifically the collection ditch at the base of the cut slope was not stabilized, and the runoff conveyed by the ditch was generating and transporting sediment off-site.  In addition there were erosion rills developing off of the well pad onto the fill slope which were alos generating and transporting sediment.

| Met onsite with operator representative Harry Mills to review the violations noted in the 02/16/12 and 03/28/12 inspections.  The issue noted with the culvert outlet immediately adjacent to the well-site had been properly addressed.  Reviewed with Mr. Mills the erosion control options to address the collection ditch on the well pad.  Mr. Mills stated that a crew will be onsite shortly to begin regrading the ditch, rocking it, and seeding and mulching the area.

| Drilling had been completed well appeared to be equipped for production.  Brine tank located onsite and it appeared to be in sound condition.  Well-site had been regraded and the vegetative cover met the 70% uniform coverage criteria.  There was no observed accelerated erosion or sedimentation in relation to the access road or well-site.  The collection ditch flowing from the end of the access road along the bottom of the well-site cut slope had been stabilized with clearn durable rock.

Inspections Performed

Inspection ID Inspection Category Inspection Date Inspection Type Inspection Result Inspection Comments
1974150 Primary Facility 2011-05-18 Routine/Complete Inspection Violation(s) Noted
1978365 Primary Facility 2011-06-03 Compliance Evaluation No Violations Noted Drilling was not ongoing, drillers were waiting to cement the string of casing that they had just finished placing.
2043356 Primary Facility 2012-02-16 Routine/Complete Inspection Violation(s) Noted
2054520 Primary Facility 2012-03-28 Routine/Complete Inspection Outstanding Violations - No Viols Req'd Drilling was completed on 06/10/11, well appeared to be equipped for production.
2062705 Primary Facility 2012-04-26 Routine/Complete Inspection Outstanding Violations - No Viols Req'd Met onsite with operator representative Harry Mills to review the violations noted in the 02/16/12 and 03/28/12 inspections.
2064308 Primary Facility 2012-05-02 Routine/Complete Inspection Outstanding Violations - No Viols Req'd Erosion control measures as discussed with Mr. Mills during the 04/26/12 inspection had not been implemented as it was discussed during the 04/26/12 site meeting. Site had been mulched, however the collection ditch flowing from the end of the access road along the backside of the well-pad had not been stabilized with rock as it was discussed during the 04/26/12 site meeting.
2085649 Primary Facility 2012-07-23 Compliance Evaluation No Violations Noted Drilling had been completed well appeared to be equipped for production.
2374986 Primary Facility 2015-05-27 Routine/Complete Inspection No Violations Noted Well appeared to be equipped for production. Brine tank located onsite and it appeared to be in sound condition. There was no observed accelerated erosion or sedimentation in relation to the site or access road.
