| At the time of inspection, a rig is set up at the well to clean out.  Well is in the middle of a golf course and access is across or near several fairways.  Top of bridge was tagged at 1391'.  There is 4 7/8" casing and 6 1/4" casing in well.  Well will be cleaned out and an attempt will be made to pull 4 7/8" casing.

| At the time of inspection, 4 7/8" casing is being worked out of the well.  Packer was worked for a day and didn't move, but next day an attempt was made again and it started to move.  The pipe is being cut with a torch and laid out on a trailer.  Well was logged and there was 750' of 4 7/8" in well.  There are 20 joints remaining to pull.

| At the time of inspection, 4 7/8" has all been pulled from well.  A few pieces of the packer broke off and fell in the well, but they should be easily drilled out.  Water was tagged at 68' down.  Plan is to bale well and set a packer at around 811-816' with 4 1/2" pipe in a 6 1/4" hole to cut off water zones.  Then remaining water will be baled and will be able to clean well out easier.

2632067 Primary Facility 2017-08-28 Plugging(Includes Plugged/Mined Through) No Violations Noted At the time of inspection, a rig is set up at the well to clean out. Well is in the middle of a golf course and access is across or near several fairways. Top of bridge was tagged at 1391'. There is 4 7/8" casing and 6 1/4" casing in well. Well will be cleaned out and an attempt will be made to pull 4 7/8" casing.
2636404 Primary Facility 2017-09-06 Plugging(Includes Plugged/Mined Through) No Violations Noted At the time of inspection, 4 7/8" casing is being worked out of the well. Packer was worked for a day and didn't move, but next day an attempt was made again and it started to move. The pipe is being cut with a torch and laid out on a trailer. Well was logged and there was 750' of 4 7/8" in well. There are 20 joints remaining to pull.
2636809 Primary Facility 2017-09-13 Plugging(Includes Plugged/Mined Through) No Violations Noted At the time of inspection, 4 7/8" has all been pulled from well. A few pieces of the packer broke off and fell in the well, but they should be easily drilled out. Water was tagged at 68' down. Plan is to bale well and set a packer at around 811-816' with 4 1/2" pipe in a 6 1/4" hole to cut off water zones. Then remaining water will be baled and will be able to clean well out easier.
2642118 Primary Facility 2017-09-20 Plugging(Includes Plugged/Mined Through) No Violations Noted At the time of inspection, well has been cleaned out to 1420'. Based on an old record of the well, expected bridges will probably be encountered at least to a depth of 1600'. Well is still making some water and needs baled before clean out takes place.
2644844 Primary Facility 2017-09-28 Plugging(Includes Plugged/Mined Through) No Violations Noted At the time of inspection, well has been cleaned out to 1518'. Well was making about 4 gallons of water an hour. Several plastic totes have been brought onsite to handle water when baling. The large open top tank remains as well. Plugging operator had to swap out cable line on rig which delayed the clean out some.
2647382 Primary Facility 2017-10-03 Plugging(Includes Plugged/Mined Through) No Violations Noted At the time of inspection, well has been cleaned out to 1666'. There were some cave-ins over the weekend which had to be cleaned out. Cuttings are being put into an open barrel and then moved to tanks onsite. Will continue to clean out.
2652308 Primary Facility 2017-10-19 Plugging(Includes Plugged/Mined Through) No Violations Noted At the time of inspection, clean out continues to take place. Currently, plugging operator has been baling water out of the well and will continuing to drill tomorrow. Depth was at 2340'. There are different records on the well showing a TD. It could be down to 2800', may request attainable bottom if bridge plug is worked a full day and not broken through. Bit was redressed to 3 7/8" and has already worn down to 3 1/2".
2655441 Primary Facility 2017-10-27 Plugging(Includes Plugged/Mined Through) No Violations Noted At the time of inspection, clean out continues to take place. Currently, plugging operator has been baling water out of the well and will continuing to drill tomorrow. Depth was at 2340'. There are different records on the well showing a TD. It could be down to 2800', may request attainable bottom if bridge plug is worked a full day and not broken through. Bit was redressed to 3 7/8" and has already worn down to 3 1/2".
2662205 Primary Facility 2017-11-09 Plugging(Includes Plugged/Mined Through) No Violations Noted At the time of inspection, a cement plug has been set from 2672' up to 850'. The 4 1/2" packer was pulled free and pulled up. Cemented through 4 1/2 with tubing. Cement top will be tagged tomorrow and more cement will be added to approx. 300'. It is unsure what depth the surface casing is set to so this depth was agreed upon to cement to.
2663257 Primary Facility 2017-11-14 Plugging(Includes Plugged/Mined Through) No Violations Noted At the time of inspection, well plugging has been completed. Rig was moved off of location this morning. A dozer remains onsite for reclamation. Cement was tagged from last job at 933'. Cement was added from here up to 282', where it was tagged. Pea gravel was added to surface. A plate was placed over well with the permit number welded on to it.
