| Admin inspection as a result of a report of a brine tank spill.  Larry Fetterman of Alliance Petroleum Corporation notified DEP OGI Paul Wrywas who took the initial information on 05/06/14. Approx, 35 barrels of brine tank content was released. Valve was opened and plug was pulled out of tank. Water truck, excavator, and disposal tanks called in for remediation. Also received a call from Ed Dziedzic of Rettew Associates who is working along with Larry Fetterman with remediation efforts. Violations may be forthcoming upon further information gathering and a field inspection.

The SCR was approved by ECB on 7-22-15. At the time the violation issued for the release was closed, the impacted soils had been excavated and removed from the site.

Valve was opened and plug was pulled out of tank. Water truck, excavator, and disposal tanks called in for remediation. Also received a call from Ed Dziedzic of Rettew Associates who is working along with Larry Fetterman with remediation efforts. Violations may be forthcoming upon further information gathering and a field inspection.

AR stabilized.As the Remedial Action Plan Completion Report was approved by Dept on 5/13/15, viols issued under record 2269834 (enf ID 310934) for reportable release of brine water have been corrected.Dept recommends Opr continue to monitor/maintain site to ensure >70% uniform and adequate veg is attained for final perm stabilization of the site upon backfilling of the site.Upon perm stabilization and final restoration of the site, all temp BMPs shall be properly removed.A follow-up inspection is anticipated to be conducted in the future to ensure site is properly restored as per current regulations. Violations may be forthcoming pending future inspections monitoring site restoration and permanent stabilization.
2554711 Primary Facility 2017-01-18 Routine/Partial Inspection Violation(s) Noted Routine parital inspection conducted to assess site remediation after a reportable release of brine at this site. Violation was issued under inspection record #2269834 and corrected under inspection record #2393173 on 7-22-15. This release occurred while the well was in production. The Operator was previously In Compliance with Policy since Operator appeared to be following procedures outlined in the Department’s document: Addressing Spills and Releases at Oil and Gas Well Sites or Access Roads, (Doc. #800-5000-001) by submitting a 15-day response on 5-23-14 and a SCR on 5-21-14. The SCR was approved by ECB on 7-22-15. At the time the violation issued for the release was closed, the impacted soils had been excavated and removed from the site.

At the time of this inspection, the area of excavation does not appear to be backfilled therefore, the area previously impacted by the release does not appear to be restored to pre-release conditions in accordance with 58Pa. C.S. § 3216(a) and is not permanently stabili

2569009 Primary Facility 2017-03-08 Administrative/File Review Outstanding Violations - Viols Req'd Admin inspection to inspection record #2554711 where violations were issued for failure to restore disturbed area of a well site and for failure to have site permanently stabilized and protected from accelerated erosion and sedimentation upon final completion of earth disturbance activity.

Department received response to NOV from Operator. In this response, Operator states that the previously excavated area will be backfilled, fertilized, seeded and mulched as soon as weather permits.

A follow up inspection will be conducted at a later date.

Previously issued violations still exist for this site.

At time of inspection no E&S issues observed on gated access road or on site. It appears site has been backfilled, seeded, and mulched. Compost filter sock installed along south edge of backfilled area. Department recommends Operator remove temporary BMP's when site has reached permanent stabilization. Previously issued violations still exist for this site due to the lack of vegetation on previously disturbed area and the site failing to reach permanent stabilization. A follow up inspection will be conducted at a later date.

At the time of this inspection, vegetation is growing but has not yet reached >70% uniform growth. However, Operator appears to be in compliance with schedule at this time. Department recommends Operator remove temporary BMP's when site has reached permanent stabilization. Violations issued under inspection record #2554711 have been corrected.

2626307 Primary Facility 2017-08-14 Routine/Complete Inspection No Violations Noted Well appears equipped for production. A reportable release of brine occurred at the site on 5-6-14 and the related violation was closed on 7-22-15. A violation was issued on 1-17-17 for failure to restore the site after the above mentioned reportable release. That violation was closed on 4-24-17 due to operator being In Compliance with Schedule. At the time of this inspection site does not appear to be permanently stabilized and does not appear to be restored. Department recommends operator reseed and mulch to ensure site reaches permanent stabilization. Follow up inspection will be conducted at a later date.
