Production equipment consisted of separator (180 psi), data logger (150 psi/0 psi), and 100 bbl brine tank (20 bbl fluid). Site has not been properly restored. Pictures of the site and request for WQ to follow up with additional inspection. No E&S issues and no IW issues during inspection surrounding the production well. Well tag present. No signs of gas leak around well. Meter present for resident.

Drilling was complete August 5, 2008, the 9 month restoration period has expired. Access road was ok. Brine tank appeared sound. Site was not adequately and uniformly restored, also noted in O&G Inspectors report (ID1887087) conducted on 5/21/10. Less than 70% successful vegetative growth was exhibited and some accelerated E&S issues to the right of well pad, as well as ponding of water along grub/brush line was observed. It is recommended that operator reseed and mulch the site in order to properly, permanently restore the site and prevent any further E&S concerns. A second follow-up inspection will be conducted in the near future to inspect corrective measures taken. 589312
