1817405 2009-07-31 No Violations Noted Currently, well completed to a depth. Well site was identified with well sign. However, name of well was correct but permit number did not match site number. Slight visible E&S concerns at the time of inspection with sediment occurring on the road and no filter fence surrounding the pad site (not included in E&S plan). No IW discharge issues. Pit aligned with 20 mil plastic and had no freeboard issues. Rig removed from site. Talk to Dorso representative about heavy rains filling pit. He was going to start removing water on 8/1/09. Plans on fracturing well on Tuesday.
1887071 2010-05-21 No Violations Noted According to records, well completed on 7/22/09 to a depth of 3,613 ft. Active permit since 7/22/09. Well head completed with 8¿ pipe reduced to 4¿ and reduced to 2¿ production line with 4 shut off valves. Production equipment consisted of separator (180 psi), data logger tank. Data logger showed pressures of 160 psi/40 psi. No E&S issues and no IW issues during inspection surrounding the production well. Well tag present. No gas leak around well.
1888996 2010-06-03 No Violations Noted At the time of inspection, the well properly tagged. Well in production. Brine tank appeared sound. No accelerated erosion or sedimentation issues were observed on site. Access road was stable. Operator should continue to monitor culvert outlets for leaf and debris build up. Site was adequately and uniformly restored.
