Although unable to remove ES plan from ¿tube¿ due to rust. Recommend operator replace tube for ease of ES plan removal. Access road adequately rocked, although department recommends that additional rock be added to culvert outlets to prevent a possible ES concern with roadway runoff. Filter fence installed down gradient of well site, although noted that filter fabric appeared to improperly installed around stakes, operator should reinstalled filter fence according to manufacture standards to prevent a possible pollution event. Additional bmp¿s should also be installed surrounding soil pile. One below grade, unlined pit onsite, presence of in pit noted. A follow up inspection will be conducted to evaluate measure taken to reduced potental ES concerns as well as bmp¿s installation.
1963216 2011-03-30 No Violations Noted Well Id sign and E&S plan present at access road. At the time of inspection, no E&S or IW discharge issues at the site. No to little mud located on paved highway at entrance way. Pit covered with 20 mil plastic. Filter fence present on the down gradient side of site. Spud called on 3/24/11. Drilling started on 3/28/11. Conductor pipe installed to 30 ft. (20¿) pipe (sand). Whipstock (Chris) set 1st stringer (13 3/8¿) pipe to 168 ft used 135 sks of class A and plug down 9:42 pm. Current depth was 1120 ft. Stratigraphy consisted of ss, sh and coal. Water encountered at 210 and 385 ft. No freeboard issues. Superior cement
1965614 2011-04-07 No Violations Noted At the time of inspection, well sign and Es plan present. According to plan operator intends to encapsulate cuttings onsite, as well as discharge top hole water in accordance with regulations. Access road stable and culverts adequately maintained. Well drilled not yet equipped for production. Present on site Paul Mony (Susquehanna Rep) and Hillsdale Construction Rep. Drill pit has been encapsulated. All bmp¿s are installed according to plan. Discussed bmp¿s and installation methods with operator. No accelerated erosion or sedimentation issues noted, nor IW issues. Operator should continue to monitor/maintain all temporary bmp¿s until final permanent restoration activities have been completed. Operator anticipates well will be fractured in 2weeks. Operator also relayed to department that Hasting Boro water supply, which is a water well, Park 1 well (~1527ft) had pre drill testing conducted and dept recommended to conducted post frac samples as well.

Pits have been removed and previously disturbed areas have been seeded and mulched, although less than 70% vegetative growth observed. Temporary bmp¿s installed and properly functioning. No evidence of accelerated erosion or sedimentation issues noted.
