| Well Id sign and E&S plan were present at access road.  At the time of the inspection, no E&S concerns and no freeboard at the pit were present at the site.  Filter fence was located on down gradient side of site.  Phillips (Kline ¿ tool pusher) was contracted by NTS.  Mine shaft was encountered at 110 feet to 115 feet interval.  Mine casing (13 3/8¿ pipe) was set at 142 feet with cement basket at 60 feet.  100 sacks of Type 1 were used to cement mine casing with 15 bbls of return on the back side of pipe.  Plug was down at 12/21/11 at 04:18 hrs.  Phillips was waiting the 8 hour cement set up time.

E&S plan shows the pit being constructed on the cut side of the pad, however it was actually built on the fill side.  These changes need to be addressed on the site plan with red pen and initialling.  This was discussed with Steve Phillips (Owner).
 
Site was rocked, slightly muddy but stable.
 
The pit was constructed nearly 15 feet below the well pad due to rock on site, according to Mr. Phillips. 
 
A good deal of springs are around the site.  After discussing these matters with Mr. Phillips, it was agreed upon to remove the cuttings for disposal rather than encapsulation.
 
BMPs were properly installed and working adequately. No E&S issues.
 
No violations noted at this time.

Mine casing (13 3/8¿ pipe) was set at 142 feet with cement basket at 60 feet. 100 sacks of Type 1 were used to cement mine casing with 15 bbls of return on the back side of pipe. Plug was down at 12/21/11 at 04:18 hrs. Phillips was waiting the 8 hour cement set up time.
2029945 Primary Facility 2011-12-28 Drilling/Alteration No Violations Noted Well Id sign and E&S plan were present at access road. At the time of the inspection, no E&S concerns and no freeboard at the pit were present at the site. Filter fence was located on down gradient side of site. Phillips (Kline ¿ tool pusher) was contracted by NTS. Conductor pipe (20¿ pipe) was set at 28 feet and was sanded in. Mine shaft was encountered at 110 feet to 115 feet interval. Water intervals were recorded at 29 feet and 300 feet. Mine casing (13 3/8¿ pipe) was set at 142 feet with cement basket at 60 feet. 100 sacks of Type 1 were used to cement mine casing with 15 bbls of return on the back side of pipe. Intermediate casing (7¿ pipe) was set at 969 feet and was cemented with 360 sacks of Type I with 20 bbls of return. Plug was down at 3:30 pm at 12/27/11. Phillips was currently drilling depth of 1,090 feet. The anticipated DTD was 3,550 feet.

2376010 Primary Facility 2015-06-01 Follow-up Inspection Outstanding Violations - No Viols Req'd On-site inspection conducted as follow-up to inspection record 2353814 (enf ID 321122) for failure to install well tag in a legible, visible, and permanent manner and for failure to submit a well site restoration report.At time of inspection, well appears drilled and equipped for production. Drilling occurred 12/19/2011-12/29/2011. Brine tank appears sound, located within an earthen dike. Well tag present, legible.Access road is developing ruts, therefore the Department recommends the Operator continue to monitor and maintain the access road as necessary in order to prevent any potential pollution events from occurring. Well-tender appeared to utilize access road during the Dept’s inspt.
2376131 Primary Facility 2015-06-03 Administrative/File Review Outstanding Violations - No Viols Req'd Admin Inspection in relation to inspection conducted on 6-1-15 with WQS S. Foulk. Department called Operator Steve Phillips by phone on 6-3-15 and left a message. Mr. Phillips returned call the same day. Department recommended removal of temporary BMP’s; stated that access road was developing ruts; and that site restoration report was not submitted. Mr. Phillips responded that he would take care of all issues in a timely manner and would call when they were addressed. He remarked that he did not use the top section of the access road, but entered the site by the bottom road. Mr. Phillips inquired about well tag and Department responded that the well tag was present and legible and permanently affixed therefore, in compliance. Outstanding violations still exist for this site (reference Inspection #2353814). Department stated that a follow-up inspection would be conducted in approximately one week.
2383878 Primary Facility 2015-06-29 Administrative/File Review Outstanding Violations - No Viols Req'd Admin inspection in response to follow-up phone call on 6-29-15. Department contacted Operator by phone and recommended Operator remove and dispose of uninstalled temporary BMP’s on site and remove temporary BMP stakes. Department recommended maintaining access road. Operator stated he was waiting on a ‘mini-excavator’ to aid in removal of BMP’s and it should be on site soon. Department stated a follow-up inspection will be conducted in about 10 days. Previously issued violations are outstanding for this site.
2383907 Primary Facility 2015-06-17 Administrative/File Review Outstanding Violations - No Viols Req'd Admin inspection to follow-up phone call to inspection #2376131. Department contacted Operator by phone on 6-17-15 and left message. Operator returned call later in day. Department stated a follow-up inspection would be conducted later in the day to assess compliance. Operator stated his workers would not be on site until 6-22-15. Department may wait until after 6-22-15 to follow-up. Previously issued violations for this site are still outstanding.

Department contacted Operator by phone on 8-4-15 in regards to follow-up site inspection and previous inspections #2383907, 2383878 and 2376131. Discussed removal and disposal of temporary BMP’s and maintenance of access road. Operator stated he would call when recommendations have been met.
