| Department representative Sue Herzing conducted a routine site inspection at BKV Operating, LLC Cowfer 1 Pad  on April 27, 2018 with environmental consultant Kevin Brady. There is one Marcellus well (027-21501) drilled on this pad. The well is currently in regulatory inactive status and this status will expire on 8/21/18. The well head is tagged. The well cellar is filled in with gravel.

The construction of this well pad and access road were issued under an E&S plan and not an ESCGP. There were 2 pits that have been back filled and restored. Gravel remains throughout the pad. No E&S issues were observed. The pad and access road appear stable. No production equipment is onsite. A non-coal mining operation borders this well pad.

2724313 Primary Facility 2018-04-28 Routine/Complete Inspection No Violations Noted Department representative Sue Herzing conducted a routine site inspection at BKV Operating, LLC Cowfer 1 Pad on April 27, 2018 with environmental consultant Kevin Brady. There is one Marcellus well (027-21501) drilled on this pad. The well is currently in regulatory inactive status and this status will expire on 8/21/18. The well head is tagged. The well cellar is filled in with gravel.

The construction of this well pad and access road were issued under an E&S plan and not an ESCGP. There were 2 pits that have been back filled and restored. Gravel remains throughout the pad. No E&S issues were observed. The pad and access road appear stable. No production equipment is onsite. A non-coal mining operation borders this well pad.

I have contacted Dave Goss, a Department surface mining inspector regarding encroachment on this well pad. Mr. Goss and I will be conducting a joint inspection together regarding this concern.

No violations noted.

There is 1 inactive well on this pad, Regulatory Inactive Well status expires on 08/21/2018. Annulus vent (9-5/8¿ x 5-1/2¿) is open and vented at time of inspection. 100% LEL and 100% combustible gas was detected at the annulus. Was not able to verify pressures during inspection. Company operator is working on filing an application for extending regulatory inactive status for 12 months. Annulus vent (9-5/8 x 5-1/2) is open and vented at time of inspection. 100% LEL and 100% combustible gas was detected at the annulus. Production casing pressure is 3750 PSI, backside pressure is 3750 PSI, annulus intermediate by production pressure is 100 PSI. Wellsite looks weedy. All equipment has been removed from the wellsite. There is 1 inactive well on this pad, Regulatory Inactive Well status expires on 08/21/2018. Annulus vent (9-5/8" x 5-1/2") is open and vented at time of inspection. 100%LEL and 100% combustible gas was detected at the time of inspection. Utilized digital manometer Dwyer Series 475 Mark III to verify the volume of gas that is venting via annulus. Calculated, using air velocity calculator, that 600 scf of gas is released daily.
