-Met with Mr. Kelly Spoonts, Drilling Consultant -Currently drilled to 6,250¿ - 7 ¿ casing set at 1,317¿ cemented w/ 350 sx cement. -Waiting on Schlumberger to repair MWD tool to resume drilling. -Derrick, substructure and BOP appear sound. -Site sign and E & S plan were on site. -E&S BMPs were in place and working adequately. -Reserve pits met regulatory standards

1823888 2009-08-13 No Violations Noted Site inspected with OGI Bruce Jankura. Also spoke with Drilling and Completion Consultant, Kelly Spoonts on site. Conductor pipe and top hole had already been drilled, drilling of horizontal section was in progress. All disturbed areas have been seeded and mulched with about 45% vegetation already grown back in. Site has brush row reinforced with line of silt fence surrounding parameter. All E&S control BMPs have been installed properly and are working adequately. The pit is constructed and lined properly. Liner appears sound with no visible cuts or punctures. More than 2ft of freeboard is being maintained
1824644 2009-08-27 No Violations Noted Site was inspected from perimeter of pad. In response to complaint #265864, Alex branch was field tested from just below McGregor Rd Crossing to headwaters. Stream begins approximately 700 Ft from edge of well pad. Meter readings indicated elevated TDS and Conductivity here. Pit appears sound. A properly installed sediment fence reinforced with haybales follows the edge of the pad. The are no signs of erosion or that any fluids have left the site on the surface. Inspection was conducted jointly with the following personel from the PA Fish and Boat Commission: Waterways Conservation Officer, Robert V. Dunbar, Fisheries Biologists, Steven Kepler and Heather Smiles. Officer Dunbar spoke with Matt Williams on-site. Mr. Williams mentioned that the well had low water returns.

1829540 2009-09-10 No Violations Noted Site was inspected with Nathan Wells, Tom McNerney and Caleb Woolever. Pit had been excavated and all recovered materials placed in 9 containers that have been covered and stored on-site until approval has been aquired to haul them to landfill. A water truck comes every other day to pull any water produced from contents. Pit has been backfilled with cleanfill. Field testing shows TDS, conductivity and pH levels in stream originating about 700' from pad have improved significantly. Casing being run on day of inspection, no E&S problems at this time

-All pits and contents removed / disposed and rough graded. -Well drilled and cased, waiting on frac job. -Water in cellar around wellhead - no bubbles seen. -No activity at report time. -Site sign and E & S plan were on site. -E&S BMPs were in place and working adequately.

1836100 2009-10-14 Violation(s) Noted Spill response: Monday, October 12, 2009 Joe Wilson, independent consultant, noticed spring behind 8H was cloudy and sudsy. EOG was notified and found that a gasket on one of the holding tanks had gone bad. approx. 190 bbls of fluids were unaccounted for. There was no indication of a spill on-site. Fluid containing about 98% fresh water, 1.06% well wash and .25% ultra vis, soaked into the pad and made its way to the nearby stream. The stream had booms placed in it and the water was being pumped out to be contained on site. Suds were seen as far down as Camp Read. EOG also flushed the area a couple of times with fresh water. They will continue to monitor the stream quality and flush if necessary. Greg Shaffer, of EOG along with Joe wilson were on site during the inspection. Representatives of the Fish and Boat Commission were also present 573592

1836167 2009-10-19 Outstanding Violations - No Viols Req'd -Site inspected with WQS Supervisor John Ryder and Geologic Trainee Caleb Woolever. Joe Wilson was conducting routine sampling at the Spring affected by the recent spill.

-Tanks are still staged on-site, there is no activity at this time -Mr. Wilson indicated that surfactants in samples were non-detect -foam was still visible in the stream -The department's samples also showed non-dectect levels, but analysis can only be run for anionic surfactants -further information gathering will occur to try to determine if the surfactants used were cationic or non-ionic and if there are any analyses to determine there presence in water.

1851110 2009-12-15 No Violations Noted - Inpsection conducted with WQS Sam Jones. Drilling complete and there are no related activities on site at this time. A small crew was on-site pumping water amongst tanks.

-Site ID sign is in place, with E&S plan in tube -All E&S controls are still in place with no signs of failure -Most of the frac tanks that had been on site for temporary storage remain. Some have been removed. The tanks on-site appear to be sound with no visible leaks. -No issues were found at this time.

1852738 2009-12-29 No Violations Noted Conducted Inspection with WQS Jessica Miller and WQS Heather Agosta.

Drilling is completed and a digital meter is connected to well. Production equipment is on location but not connected to well. Frac Tanks are on location. One tank is leaking from valve and is freezing.

1853885 2009-12-29 No Violations Noted -Inspection Conducted with OGI Travis and WQS from Altoona, Heather Agosta.

-no heavy activity has occurred since the last inspection. Production tanks have been dropped off on location -It appears that freezing on frac tank H1245 has caused some water expansion. A small amount has leaked from one of the tank outlets and froze forming an icycle about 8 inches long. due to the water being maintained on the tank this is not being considered a violation. -I contacted Nathan Wells via phone on 06Jan10 concerning the leak. He informed me that the company is working on having the brine in those tanks hauled off and will have that tank checked out and fixed.
