2048510 2012-01-27 No Violations Noted On January 19, 2012, Chad Eisenman of Chevron called WQSS Vince Yantko to report a cuttings spill at the Polk pad in Wharton township, Fayette County.

On January 27, 2012, I stopped at the Polk site to conduct an inspection. The site was well-marked and a sign with the well permits was in front. Both the ESP and the PPCP were on-site. All four wells currently drilled or spudded have individual signs.

The entrance was well rocked and the E&S controls up the sides were in place and in good shape. The bare areas were stabilized. On the pad, the sediment traps were installed as required and the diversion channels were rocked and well-maintained.

The spill area was cleaned and new rock put down.

I found no problems with the site.

2252659 2014-03-11 No Violations Noted On 3/11/2014, the Department performed an inspection of the Polk Well Units 1, 2, 3, 4, and 5 H in Wharton Township, Fayette County. The wells are owned and operated by Chevron Appalachia LLC.

The site was well marked with a Act 9 ID sign. The E&S plan, PPC plan, well permits and ERP plan were located in the mailbox at the site entrance. The access road into the site was in good shape and the entrance to the pad was gated and locked. All five wells are in. There is no other equipment on the site except for dumpsters. There are 3 dumpsters on the tank pad area and 9 dumpsters on the well pad. The 9 on the well pad are on containment. The area looks to be stable. The sed trap at the site is fairly full of water, but not running out. There was no activity at the site today. The site has been granted inactive status.

2364326 2015-04-16 No Violations Noted The site appeared recently disturbed and was seeded and mulched. Erosion was occurring along the fence between the pad and former tank farm. There was what appeared to be acid rock drainage in the area of the removed sediment trap. This area also contained a large amount of surface water. The drainage ditch the left side of the access road near the Act 9 sign also contained what appeared to be acid rock drainage. The site had four non haz waste barrels that were un labeled on the pad and two on their sides near the gate to the pad.
