| 2252455 | 2014-03-11 | No Violations Noted | On 3/11/2014, I conducted an inspection at the Chevron Appalachiaï¾’s Rejiss 1H, 2H, 3H Well Unit site in Nicholson Twp., Fayette County. The reason for the inspection was a request by the operator for an extension on the date that the site needed to be restored by. The original restoration date was 3/19/2014 and an extension to 3/19/16 is being requested.
The access road to the site was in good shape. The site was not guarded or gated off. The plans and permits were in the mailbox. One of the mailboxes has been vandalized and is missing. The ID sign is in place. The Chevron site regulations sign has been shot several times with a shot gun. All three cellars are in and the containment liner is in place on the pad. No other equipment is on site. The restoration deadline date is approved to be extended to the new date of 3/19/2016 with the understanding the operator will ensure the reseeding of the bare areas of the stockpiles and continue to monitor and minimize the offroad vehicle activity. |
