| On 1/30/2014, the Department conducted an inspection at the Chevron Appalachiaï¾’s Sanfilippo Well Unit site in Redstone Twp., Fayette County.

Access into the site was guarded.
 
 
 
The site is split between two different pad areas, 3 thru 6 has one separate pad and 1 & 2 has it's own pad.
 
 
 
Pad 3 to 6 had all the wells drilled and had a new containment pad around them.  No activity was occurring at this site today.
 
 
 
The Sed trap looked to be functioning and no E&S issues were observed.
 
 
 
The Operator needs to monitor the vegetation on the cut slope of the pad in the spring to ensure proper coverage.

On 01/04/16 at approximately 0700 hours I conducted a routine complete inspection of the site. The Act 9 sign was in place. All required documents were present at the site.

The site was not restored at the time of inspection. It appeared the pipeline connection had been recently installed to the Pad A. The site was stable.

The Department has reviewed the request for site restoration extension. The restoration date was to be 03/24/16, but a new date of 10/31/16 has been approved for both the Pad A and Pad B.

2564235 Primary Facility 2017-02-23 Routine/Complete Inspection No Violations Noted The Act 9 sign was in place and all required documents were present at the site. The site was vegetated, but additional growth is needed to reach a uniform 70% growth. Small erosion rills were observed on the site (see photographs). The rills had been addressed by Chevron and straw bale BMPs were in place to prevent further erosion. One rill appeared to have previously had straw bale BMPs present, but they had since degraded. It is recommended this location be addressed.
2693643 Primary Facility 2018-02-05 Site Restoration No Violations Noted A site restoration inspection was conducted in response to an NOT submittal by Chevron. The Act 9 sign was in place and all required documents were present at the site. The production equipment on both Pad A and pad B appeared to be in good condition. The site was well vegetated and there were no signs of erosion observed. The site meets and exceeds 70% uniform vegetative cover. The NOT is approved.
