1946821 2010-12-08 Violation(s) Noted Rick Palmer and I inspected this site for OG Dam permit application (95-7-60915-13). There are many buildings within 200 ft of the impoundment. The failure of this impoundment will impact the inhabited structures, cause serious property damage, and/or flood roadways. In case of ESCGP-1 (ESX10-059-0020), the emergency spillway of a sediment trap (near at LDZ-1) shall be revised properly to the existing bank to bank swale or stream. In addition, we found 4 inch pipelines was on the ground, on the stream bed, cross the Whiteley Creek (~30ft width), and Reverin Wetland (R5UBH, 27.46 acres) without Chapter 105 permit. See the details attached. 603863

1974880 2011-03-29 No Violations Noted The Department was at the above referenced Marcellus Well to conduct a Routine Inspection. The site is in over-all good condition. The collection channel at the toe of the fill slope needs to be addressed. The channel appears to be caving in on itself and may need some additional rock placed in the channel to keep any sediment out of the private pond directly down-gradient.
