| On Friday, September 8, 2017, I conducted a routine-complete inspection of the Pettit Well Site located in Washington Township Greene County.

The Act 9 sign and a separate sign with API permit numbers were in place a clearly legible at the site entrance.  All required documentation was available on-site.

The access road and well pad and impoundment were in good condition.  Overall, the site is well vegetated and exhibits uniform perennial growth throughout the majority of the site.  There are a number of area with less than uniform growth.  Some areas around the impoundment appear to have been worked relatively recently which would explain the lack of growth.  Other areas around the site appear to have bedrock outcropping which will definitely not support growth.  The might consider re-seeding areas of minimal growth with the exception of those areas with bedrock outcropping.

The Act 9 sign and a separate sign with API permit numbers were in place a clearly legible at the site entrance. All required documentation was available on-site.

The access road and well pad and impoundment were in good condition. Overall, the site is well vegetated and exhibits uniform perennial growth throughout the majority of the site. There are a number of area with less than uniform growth. Some areas around the impoundment appear to have been worked relatively recently which would explain the lack of growth. Other areas around the site appear to have bedrock outcropping which will definitely not support growth. The might consider re-seeding areas of minimal growth with the exception of those areas with bedrock outcropping.

The secondary containment structures on-site appear to be in good condition and functioning as intended. The gas wells appear to b

2732971 Primary Facility 2018-05-04 Routine/Complete Inspection No Violations Noted On Friday, May 4, 2018, I conducted a routine-complete inspection at the Pettit Well Site located in Morris Township, Greene County.

The Act 9 sign was clearly legible at the site entrance.

All required documentation was available on-site.

The access road, impoundment pad and the well pad were in good condition.

The site has been stabilized with a combination of vegetation and E&S controls.

The impoundment appeared to be in good condition and the water level was below freeboard.

The secondary containment around the production tanks appeared to be in good condition and functioning as designed.

Gas well 591372 and the associated infrastructure appeared to be in good condition and functioning properly. The gas well was labelled.

No violations were identified during the inspection.

2815508 Primary Facility 2018-12-13 Routine/Complete Inspection No Violations Noted On Thursday December 13, 2018 I conducted a routine-complete inspection at the Pettit Well Site located in Washington Township, Greene County.

The gas well and the associated infrastructure appeared to be in good condition and functioning properly. The gas well was labelled with identification.

No violations were identified during the inspection.
