The new Act 9 Site ID sign was in place and legible. The E&S plan was on site with the site foreman, Bill Balog. The well pad, and access road were completed and all E&S controls were completed. A landslide outside of the LOD which had filled a drainage ditch along the upslope of the access road, has been cleaned out. All soil stockpiles were stable and recently seeded and mulched. |

| 2231366 | 2013-12-04 | No Violations Noted | On 12/4/2013, I performed on inspection of EQT's Lacko well pad site. The site is located in Washington Twp., Greene County. The inspection was performed as a routine inspection during drilling.
The new Act 9 Site Id sign was in place and legible. The well pad, access road and all E&S controls were completed and funtional along the access road, well pad, and impoundment. The fresh water impoundment was being filled in preparation for stimulation in the future. Drill rig was not on site but operator informed me that the rig would be onsite in two weeks to begin drilling the laterals. The top holes have been drilled already. The slopes at the the site were vegetated and stable and the newly constructed fresh water impoundment, and soil stockpiles were stable and mulched.
