| On 9/4/2013, I performed an inspection of EQT Shipman well pad site.  The site is located in Washington Twp., Greene County.  The inspection was performed due to a request by the Operator for a Pre-operational inspection.

The new Act 9 Site ID sign was in place and legible. The  E&S plan was on site with the site foreman, Rex Nelson.

The well pad, and access road were being constructed but all E&S controls were completed.

There were several changes made to the E&S plan which were corrected on the site plan.  A sediment trap on the south side of the soil stockpile on the west side of the site was eliminated and the sediment trap on the south side of the fresh water pit was redesigned and constructed to handle the new water diversion.  I informed the operator to monitor the newly designed E&S control for effectivness.  Two pad fresh water drains were added for stability of the drill pad.

All soil stockpiles were stable and in process of being seeded and mulched.

The new Act 9 Site Id sign was in place and legible.

The well pad, access road and all E&S controls were completed and funtional along the access road, well pad, and impoundment.  The fresh water impoundment was being filled in preparation for stimulation in the future.

Drill rig was being setup on site to begin drilling the laterals.  The top holes have been drilled already.

The slopes at the the site were vegetated and stable and the newly constructed fresh water impoundment was being seeded and mulched, and soil stockpiles were also stable and mulched.
