1865725 2010-02-24 No Violations Noted According to records, permit granted 8/21/09. Access road on the steep grade was draining. Area behind pits was draining onto the pad creating a wet zone on the pad. DEP requested that the WQ inspect site for drainage issues. At the time of inspection, Well Superior was cementing the 9 5/8¿ pipe which extended to 250ft. Conductor pipe was set at 18 ft. Anticipated TD is 3940 ft. Pit aligned with 20mil plastic and no freeboard issues. E&S controls are in place. At the time of inspection, no IW and E&S issues at the site.
1865871 2010-02-25 No Violations Noted Additional access road road BMP's are being installed. Site E&S ok. Pit liner was to be anchored better on the backside of the site due to the expected upcoming weather conditions. Plow and broom truck located onsite to address andy further off-site transport of sediment. Site and access road should be temporarliy stabilized with the additional road BMP's until the rig is moved off-site
1867378 2010-03-03 No Violations Noted According to records, permit granted 8/21/09. Problem occurred with poor cementing of the first stringer. Hence, PC called Rick Hoover (DEP) and confirmed to skid rig 15 ft north. Well was plugged with paperwork (intent to plug and certificate submitted to DEP). Well #2A was drilled depth of 3940 ft. Conductor pipe set at 17¿; 9 5/8¿ set to 331 ft. and 7¿ set to 1351¿. Currently, extra pipe was being removed from site and wire line study was being performed. Site pad was heavily saturated causing vehicles to be moved with dozer. Abundant amount of mud was located on the township road. Slight odor of gas noted from well. Upon completion of wire line, Well Superior was going to cement final section. Pit aligned with 20mil plastic and no freeboard issues. E&S controls are in place. At the time of inspection, no IW and E&S issues at the site. Requested PC to clean mud from road upon completion of project.
