Recommended that rock be added to ditch surrounding soil pile in order to reduce the potential or accelerated erosion and sedimentation concerns after rain events. Pad actively being rocked. No accelerated erosion or sedimentation issues noted during this inspection. Operator shall continue to closely monitor and maintain all bmp's until final permanent restoration achieved.

2072375 2012-05-24 In Compliance with Policy Admin inspection- At approximately 830 I received report from Duty Office Vince Yantko of reportable release of diesel to secondary containment from a leaking filter. It appears that 3 1/2 barrels of diesel fuel leaked onto secondary containment. Operator plans to collected material and dispose of according to regulation. It appears operator is complying with current spill policy. As spill was promptly reported and immediate remedial actions were taken. OGI Rick hoover plans to follow up with a site inspection on this reportable release on Friday May 25, 2012.
2072377 2012-05-29 In Compliance with Policy Admin inspection- At approximately 1227 I received a message from Nick Staffieri (Chevron) of a 1qt oil spill onto ground from water truck on location. Operator plans to visual remove oil and impacted soils from pad and place in contained and properly dispose of according to regulator standards. Based on current spill policy, it appears operator is in compliance with current spill policy and is reporting (the release) the proper measures are being taken to contain/remove/dispose of release.

Spoke with Curtis Small (Chevron) whom stated he would send operator incident report and closure upon final investigation of release.

Based on reported information, it appears operator is in compliance with current spill policy.

Access road in good condition. No accelerated erosion or sedimentation issues observed in relation to well site or access road. One "freshwater" impoundment on-site, surrounded by orange security fence, nearly full. Department recommends Operator closely monitor and maintain impoundment as necessary in order to prevent a potential pollution event from occurring should "freshwater" breech impoundment.

Based on Operator's written request and this site inspection, an extension for site restoration will be granted until December 7, 2014.

2252520 2014-03-11 In Compliance with Policy Spill reported by Operator representative B A Lambie to SWRO at 1419. Call taken by Lynne Rupp (See Attached).

Five gallons diesel fluid to ground at 1300. Already cleaned-up at time of call. No waterways affected.

Operator appears to be in compliance with current policy at this time.

2252867 2014-03-11 In Compliance with Policy On-site inspt in response to admin inspt 2252520 for reported release of 5 gal diesel fluid to ground.

According to on-site Operator representative Gene, a rock knocked the fuel valve off machinery in transit that was seeding/mulching the site (restoration activities currently underway), subsequently causing the diesel fluid to spill onto well pad. Valve was immediately plugged, the diesel fluid contained, and absorbent pads placed on all impacted materials. Impacted gravel promptly excavated and placed into drums to await proper disposal. Spill occurred at approximately 1300 and reported to be cleaned-up within 20-30 min.

Pictures shown to Department to illustrate impact of release and clean-up measures taken. Release did not travel off well pad.

At time of inspection, no visibly impacted materials remain, no diesel odor noted, and no adverse environmental impacts as a result of this event observed.

Operator appears to be in compliance with current policy at this time.
