1802948 2009-06-03 No Violations Noted Based on file records, well was completed on 4/23/59 to a depth of 8,130 ft. from grade and maintains an active permit status. During inspection, no proper well tag was present at well head and site restoration was very limited vegetation growth along with poor access road entry. According to my conversation with Karen Reeseman (Dominian). A well tag was going to be placed on the well head during week of June 8th. Well production equipment was removed from site and well abandoned. Dominian applying for inactive status. DEP requested from Dominian the following information: type of cement used during the 1959 well installation, current wellhead pressure, last production date of this well. Dominian was also going to look into the current access road condition and site restoration according to Ms. Reesemen. During the time of inspection, no E&S and IW discharge to ground issues at the site.
