Recommend that operator add additional rock to access road in order to prevent further erosion. Well drilled, not yet fractured. Secondary containment pad being installed over well pad. Forty-two structurally sound frac tanks located on well site. Installed BMPS appear to be maintained and functioning correctly. No accelerated erosion or sedimentation issues noted on site. Recommend operator remove ¿garbage pile¿ located on right back side of pad (when facing well head from access road). All previously disturbed areas have been stabilized with vegetation. Accompanied by WQS Marc Maestra during inspection. 
 
  
Note: Upon our arrival, Mick Planinsek -Soild Waste Specialist for Bureau of Waste Management located on well pad discussing a complaint filed with DEP regarding a leaking Keystone Vac truck ¿believed¿ to have left this well site-unknown what truck was hauling.

Operator should continue to routinely monitor and maintain all bmp¿s until permanent restoration has been achieved. No accelerated erosion or sedimentation issues noteD onsite or access road. | Inspection the result of the Operators’ request to discuss the restoration of the well pad area.  At the time of inspection the site appeared to be in sound condition.  The E&S plan was onsite and appeared to be implemented properly.  All BMPs appear to be in installed and maintained as outline in the plan.  No accelerated erosion or sedimentation observed in relation to the access road or well pad area.  The Departments suggests the Operator maintain the perimeter control while restoring the site and inspect weekly and after significant precipitation event to ensure the continued stability of the site.  The Department notes that the restoration plan calls for the restored cut and fill slopes of 3:1 and the dispersal of the top soil stock pile, however the approved plan does not call for the reduction in the size of the well pad.
 
| Inspection the result of the Operators’ request to discuss the restoration of the well pad area.  At the time of inspection the site appeared to be in sound condition.  The E&S plan was onsite and appeared to be implemented properly.  All BMPs appear to be in installed and maintained as outline in the plan.  No accelerated erosion or sedimentation observed in relation to the access road or well pad area.  The Departments suggests the Operator maintain the perimeter control while restoring the site and inspect weekly and after significant precipitation event to ensure the continued stability of the site.  The Department notes that the restoration plan calls for the restored cut and fill slopes of 3:1 and the dispersal of the top soil stock pile, however the approved plan does not call for the reduction in the size of the well pad.

1969231 Primary Facility 2011-04-29 Routine/Complete Inspection No Violations Noted During inspection, valid site Id sign, ES plan, ESCGP-1, and sewage permit (waste holding tank) located onsite. Well spud updated to 4/25/11. Noted that PPC plan contacts were not updated, specifically those within the DEP. 24hr security guard located onsite. Rig located onsite actively drilling well. No pits located on pad, operator utilizing closed containment tanks, which appear structurally sound with freeboard. Operator installed ¿containment pad beneath rig¿ in order to prevent a possible pollution event. Noted a wood chip pile located on right had side of rig, no industrial waste or discharge detected during inspection, although recommended operator further investigate pile. remove drill piping from sediment ditch, in order to prevent a possible pollution event. Sediment ditches and traps appear adequately rocked and surrounding disturbance temporally seeded/mulched, but minimal growth observed. No accelerated ES issues. Operator should continue to routinely monitor/maintain all BMP¿s until final restoration is completed.
