Pad actively being re-rocked. Frac (empty) tanks present onsite; noted several with bottom lids open. Recommend operator close tank links in order to prevent a possible pollution and/or safety incident. All previously disturbed areas have been seeded and mulched with growth observed. BMPs installed and functioning, appears sediment trap is full. Operator should continue to monitor and maintain ALL BMPs until final restoration is complete. No accelerated erosion or sedimentation issues noted.

2016702 2011-11-01 Violation(s) Noted Inspection result of reported incident received by myself (as well as Marc Maestra) via phone call on Tues November 1, 2011 at 1050am from Chad Eisenman of Chevron. He called to provide notification of a discharge/release of an undetermined amount of flow back on to the grounds of the commonwealth. He stated that it is believed that secondary containment located beneath frac tanks overflowed. At the time of phone call no further data was available.

At the time of inspection, Chevron Regulatory Compliance personnel onsite, as well as KMI trucking with vac trucks. Well sign, ES plan ,PPC plan onsite. It appears that flow back (still undetermined amount) did indeed overtop secondary containment, at this time no "liquid" present on pad, several small puddles noted outside of containment and along access road. Noted minor staining on pad and roadway

2017970 2011-11-03 Outstanding Violations - No Viols Req'd follow up to operator reported incident on 11-1-11. At the time of inspection, it appears operator has removed approximately 6 inches of "pad" from areas affected by release. Contaminated soils are located on secondary containment on well pad. Department also observed various pilot test holes most likely used to determine the depth of extend of contamination--at this time visible evidence(liquid fraction or staining) of contamination not observed in holes. Operator also removed soils from access road. At the time of this inspection, it appears operator has controlled release and no "visible" evidence identified that area still contaminated by release, operator indicated plans to conduct sampling to ensure all continmated soils were removed, as well as confirm the area of contamination(width and depth). No new violations noted during this inspection, clean up of spill ongoing.Follow up inspections will be conducted to monitor and ensure clean up meaures are being dillegently proceeded with.
1969366 2011-04-29 No Violations Noted At the time of inspection, valid site Id sign, ES plan, and ESCGP-1(final approval 4/14/11), located onsite. Well spud 4/20/11. Noted that PPC plan contacts were not updated, specifically those within the DEP. 24hr security guard located onsite. Rig located onsite actively drilling well. No pits located on pad, operator utilizing closed containment tanks, which appear structurally sound with freeboard. Operator installed ¿containment pad beneath rig¿ in order to prevent a possible pollution event. No accelerated erosion or sedimentation issues observed. BMP¿s installed according to ES plan. Operator should continue to routinely monitor/maintain all BMP¿s until final restoration is completed. Access road stable.
1975381 2011-05-06 No Violations Noted At the time of inspection, no E&S or IW discharge issues at the site. Chief contracted UDI rig 5. Conductor pipe set at 32 ft using 26¿ pipe. First stringer set at 419 ft using 20¿ pipe. Universal performed cement for first stringer and used 470 sks of gas block with 20 bbl returns. Second stringer set at 13 3/8¿ to 621 feet with gas block of 20 bbl return. Third stringer set at 1791 ft of 9 5/8¿ pipe with 650 sks of gas block with 25 bbl return. Current depth set at 5000 feet. Bridge plug set at 1700 ft into the 9 5/8¿.

Chief underlain drilling rig and associated equipment with protective layer of grip plastic for the prevention of a surface release. The site is bermed around the edges of the plastic.

2046720 2012-02-22 No Violations Noted CEI inspection for reportable release of flow back water onto and outside of secondary containment on 10/01/11. At the time of this inspection, site id sign, ESCGP-1, ES & PPC plan present. Security guard located at site entrance. Access road stable no accelerated ES. Well pad appears to be restored --previously excavated areas have reclaimed as additional fill materials placed on pad. All excavated materials were placed in roll off boxes and properly disposed of according to regulation. One roll off box remains onsite.

Department unable to visibly note any remnants of spill at time of this inspection. All confirmatory sample results submited by department were below Act 2 standards, although it was noted that several samples demonstrated elevated chlorides and barium levels as a result those areas were further excavated accordingly.

2102882 2012-09-21 No Violations Noted Inspection the result of the Operators’ request to discuss the restoration of the well pad area. At the time of inspection the site appeared to be in sound condition. The E&S plan was onsite and appeared to be implemented properly. All BMPs appear to be in installed and maintained as outline in the plan. No accelerated erosion or sedimentation observed in relation to the access road or well pad area. The Departments suggests the Operator maintain the perimeter control while restoring the site and inspect weekly and after significant precipitation event to ensure the continued stability of the site. The Department notes that the restoration plan calls for the restored cut and fill slopes of 3:1 and the dispersal of the top soil stock pile, however the approved plan does not call for the reduction in the size of the well pad.

Site restored to PCSM Plan. Pad permanently stabilized. Site seeded and mulched with growth observed through light snow cover. Department recommends that Operator continues to monitor site to ensure that the required 70% vegetative cover is reached. Access road adequately rocked. No E&S issues were observed in relation to the access road or well-site.
