It appears that flow back (still undetermined amount) did indeed overtop secondary containment, at this time no "liquid" present on pad, several small puddles noted outside of containment and along access road. Noted minor staining on pad and roadway

| follow up to operator reported incident on 11-1-11.  At the time of inspection, it appears operator has removed approximately 6 inches of "pad" from areas affected by release.  Contaminated soils are located on secondary containment on well pad. Department also observed various pilot test holes most likely used to determine the depth of extend of contamination--at this time visible evidence(liquid fraction or staining) of contamination not observed in holes. Operator also removed soils from access road. At the time of this inspection, it appears operator has controlled release and no "visible" evidence identified that area still contaminated by release, operator indicated plans to conduct sampling to ensure all continmated soils were removed, as well as confirm the area of contamination(width and depth). No new violations noted during this inspection, clean up of spill ongoing.Follow up inspections will be conducted to monitor and ensure clean up meaures  are being dillegently proceeded with.

No pits located on pad, operator utilizing closed containment tanks, which appear structurally sound with freeboard. Operator installed ¿containment pad beneath rig¿ in order to prevent a possible pollution event. No accelerated erosion or sedimentation issues observed.  BMP¿s installed according to ES plan. Operator should continue to routinely monitor/maintain all BMP¿s until final restoration is completed. Access road stable.

He called to provide notification of a discharge/release of an undetermined amount of flow back on to the grounds of the commonwealth.  He stated that it is believed that secondary containment located beneath frac tanks overflowed. At the time of phone call no further data was available.

| follow up to operator reported incident on 11-1-11.  At the time of inspection, it appears operator has removed approximately 6 inches of "pad" from areas affected by release.  Contaminated soils are located on secondary containment on well pad. Department also observed various pilot test holes most likely used to determine the depth of extend of contamination--at this time visible evidence(liquid fraction or staining) of contamination not observed in holes. Operator also removed soils from access road. At the time of this inspection, it appears operator has controlled release and no "visible" evidence identified that area still contaminated by release, operator indicated plans to conduct sampling to ensure all continmated soils were removed, as well as confirm the area of contamination(width and depth). No new violations noted during this inspection, clean up of spill ongoing.Follow up inspections will be conducted to monitor and ensure clean up meaures  are being dillegently proceeded with.

2016702 Primary Facility 2011-11-01 Incident- Response to Accident or Event Violation(s) Noted Inspection result of reported incident received by myself (as well as Marc Maestra) via phone call on Tues November 1, 2011 at 1050am from Chad Eisenman of Chevron. He called to provide notification of a discharge/release of an undetermined amount of flow back on to the grounds of the commonwealth. He stated that it is believed that secondary containment located beneath frac tanks overflowed. At the time of phone call no further data was available.
2017970 Primary Facility 2011-11-03 Follow-up Inspection Outstanding Violations - No Viols Req'd follow up to operator reported incident on 11-1-11. At the time of inspection, it appears operator has removed approximately 6 inches of "pad" from areas affected by release. Contaminated soils are located on secondary containment on well pad. Department also observed various pilot test holes most likely used to determine the depth of extend of contamination--at this time visible evidence(liquid fraction or staining) of contamination not observed in holes. Operator also removed soils from access road. At the time of this inspection, it appears operator has controlled release and no "visible" evidence identified that area still contaminated by release, operator indicated plans to conduct sampling to ensure all continmated soils were removed, as well as confirm the area of contamination(width and depth). No new violations noted during this inspection, clean up of spill ongoing.Follow up inspections will be conducted to monitor and ensure clean up meaures are being dillegently proceeded with.

At the time of inspection the site appeared to be in sound condition. The E&S plan was onsite and appeared to be implemented properly. All BMPs appear to be in installed and maintained as outline in the plan. No accelerated erosion or sedimentation observed in relation to the access road or well pad area. The Departments suggests the Operator maintain the perimeter control while restoring the site and inspect weekly and after significant precipitation event to ensure the continued stability of the site. The Department notes that the restoration plan calls for the restored cut and fill slopes of 3:1 and the dispersal of the top soil stock pile, however the approved plan does not call for the reduction in the size of the well pad.

Well pad rocked, no E&S issues observed. Well head surrounded by security fence. Well tag present and legible. Well does not appear equipped for production. Operator is reminded that as per 25 Pa. Code § 78a.104, approval of inactive status for a well is valid for 5 years unless revoked. After 5 years, the owner or operator shall plug or return to active status a well granted inactive status unless the Department grants an application for a 1-year extension.
