1836956 2009-10-21 No Violations Noted The operator submitted a request for a six month site restoration extension on 10/9/09 due to pipeline and compressor station scheduling difficulties. The well reached total depth on 12/14/08 and the ID sign and E&S plan was on site. All E&S controls were installed properly and need to be maintained and inspected after every significant rainfall event. The following extension has been approved, and the deadline for the site restoration is 4/21/09. The Department requests that the fill slop be inspected on a regular basis to hinder any accelerated erosion
1910624 2010-07-28 No Violations Noted On July 28th, 2010 I conducted a routine inspection of the above site. Drilling was completed and the well was in production. The access road was in good condition. A site ID sign and E&S plan were on posted site. A well tag was present. Drilling pits had been closed. The site was graded, seeded and mulched and growth was coming in well. The well head was enclosed by a security fence. The tanks were enclosed with a lined and steel walled secondary containment structure. The E&S controls appeared to be functioning properly and should continue to be inspected routinley and after every significant rainfall event.

1959549 2011-03-10 No Violations Noted On March 10th, 2011 I conducted a routine inspection of the above site. Drilling was completed and the well was in production. The access road was in good condition. A site ID sign and E&S plan were on posted site. A well tag was present. The site was restored and had good growth. The well head was enclosed by a security fence. The tanks were enclosed with a lined and steel walled secondary containment structure. A small ditch was beginning to erode at the back of the pad due to the recent rains. The operator was notified ant crews were onsite immediately to stake hay bales at the top of the pad and line the ditch with hay to curtail any further erosion.
