19:10 hrs ¿ received 2nd call from Mike Arch Oil and Gas Inspector supervisor of fire on a Chesapeake Well site. 19:55 hrs ¿ Oil and Gas Inspector Don Rugh arrived at entrance to well site. Access road is approximately ¾ mile long. Multiple fire companies were at site, perimeter control was in place and emergency equipment was being controlled from this point. At this time I started calls to Mike Arch with updates every 15 minutes |

My purpose for being on location was to gather information from a technical perspective for the department's bureau of investigations. I observed no violations during the course of my investigation, and had the cooperation of everyone on location. My remarks on the following page detail my investigation. The following documents that I collected that day are in the possession of the bureau of investigations: Site roster at the time of the fire, tank strap information, and a timeline of events from spud through flowback up to the time of the fire.
